目的 观察三维多回波恢复梯度回波(3D MERGE)序列用于胎儿脊柱成像的价值。方法 对47胎脊柱无异常胎儿行脊柱MR二维稳态采集快速成像(2D FIESTA)和3D MERGE成像,对比 2种图像之间脊柱颈、胸、腰骶椎主观评分,以及相同序列图像中颈、胸、腰骶椎之间主观评分的差异;根据孕周将胎儿分为<28周组及≥28周组,观察其颈、胸及腰骶椎图像主观评分的差异。结果 全部胎儿3D MERGE图像显示颈、胸及腰骶椎的主观评分均高于2D FIESTA图像(P均<0.01)。2D FIESTA及3D MERGE图像显示全部胎儿颈椎主观评分均低于胸及腰骶椎(P均<0.05),而胸椎与腰骶椎主观评分差异均无统计学意义(P均>0.05)。47胎中,<28周组21胎,≥28周26胎。针对<28周组,2种图像间及2D FIESTA图像显示脊柱的主观评分结果与针对全部胎儿结果相同,3D MERGE图像显示颈、胸及腰骶椎主观评分总体差异无统计学意义(P>0.05);≥28周组2D FIESTA图像显示颈椎主观评分低于3D MERGE(P<0.01),胸及腰骶椎主观评分差异均无统计学意义(P均>0.05);2D FIESTA及3D MERGE图像显示脊柱主观评分结果与针对全部胎儿结果相同。结论 3D MERGE序列用于胎儿脊柱成像具有一定临床应用价值;与2D FIESTA序列联合应用可为临床提供更多可靠信息。 |

Objective To explore the value of three-dimensional multiple-echo recalled gradient echo (3D MERGE) sequence applied in fetal spine imaging. Methods Spine MR two-dimensional fast imaging employing steady state acquisition (2D FIESTA) and 3D MERGE images were acquired from 47 fetuses without spine abnormalities. The imaging quality of all fetuses were evaluated according to the subjective scores of cervical, thoracic and lumbosacral vertebrae and compared between 2 kinds of images, also among different parts of spine. Then the fetuses were divided into <28 weeks group and ≥28 weeks group according to gestational age, and the subjective scores of fetal cervical, thoracic and lumbosacral vertebras were hierarchically analyzed. Results The subjective scores of cervical, thoracic and lumbosacral vertebrae on 3D MERGE images were higher than those on 2D FIESTA images for all fetuses (all P<0.01), of cervical vertebrae were lower than of thoracic and lumbosacral vertebrae on 2D FIESTA and 3D MERGE images (all P<0.05). No significant difference was found between thoracic and lumbosacral vertebra (both P>0.05). Among 47 fetuses, 21 were in<28 weeks group and 26 were in ≥28 weeks group. For <28 weeks group, the subjective scores of spine between 2 kinds of images or 2D FIESTA images were the same as those of all fetuses, but there was no significant difference of subjective scores of cervical, thoracic nor lumbosacral vertebrae on 3D MERGE images (P>0.05). For ≥28 weeks group, the subjective score of cervical vertebrae on 2D FIESTA images was lower than that on 3D MERGE (P<0.01), while no significant difference of thoracic nor lumbosacral vertebrae was found (both P>0.05), and the spinal subjective scores of 2D FIESTA and 3D MERGE images were the same as those of all fetuses. Conclusion 3D MERGE sequence had certain application value for fetal spine imaging. Combining with 2D FIESTA sequence might provide further information. |
