      目的 评价电针激发SPECT脑血流灌注显像评估早期神经精神性系统性红斑狼疮(NPSLE)患者脑细胞功能储备的价值。方法 对25例NPSLE患者于电针激发前(基础状态)与电针激发状态下行SPECT脑血流灌注成像,经视觉分析和定量分析判定脑血流灌注和功能异常区,比较不同状态下全脑及各脑区血流灌注值的差异。结果 基础状态下,SPECT脑血流灌注显像于25例共见138处局灶性脑血流灌注异常和功能低下区,其中15例(15/25,60.00%)存在全脑皮质弥漫性血流灌注及功能低下;电针激发状态下病灶均有不同程度改善,完全及明显改善率为72.46%(100/138),且全脑及双侧额叶、顶叶、颞叶、枕叶血流灌注值均明显高于基础状态(P均<0.05)。结论 电针激发SPECT脑血流灌注显像能可视化评估早期NPSLE患者功能低下脑细胞及其功能储备。

      Objective To observe the value of electroacupuncture stimulated SPECT cerebral blood flow perfusion imaging for assessment of functional reserve of brain cells in patients with early neuropsychiatric systemic lupus erythematosus (NPSLE). Methods SPECT cerebral blood flow perfusion imaging was performed in 25 patients with NPSLE before (basic state) and under electroacupuncture state. Visual analysis and quantitative analysis were used to explore abnormal areas of cerebral blood flow perfusion and function, and cerebral blood flow perfusion values of the whole brain and each brain area under basic state and electroacupuncture state were compared. Results Basal state SPECT cerebral blood flow perfusion imaging detected 138 abnormal cerebral blood perfusion and dysfunctional areas in all 25 cases, while diffuse cerebral cortex blood hypoperfusion and hypofunction were found in 15 cases (15/25, 60.00%). Under electroacupuncture state, the above abnormalities were improved to various degrees, and the complete or obvious improvement rate was 72.46% (100/138). Blood flow perfusion values of the whole brain and bilateral frontal, parietal, temporal and occipital lobes under electroacupuncture state were significantly higher than those in basic state (all P<0.05). Conclusion Electroacupuncture stimulated SPECT cerebral blood flow perfusion imaging could visually assess hypofunction brain cells and functional reserve of brain cells in patients with early NPSLE.
