目的 观察首发非自杀性自伤(NSSI)青少年脑白质网络拓扑属性特征及其与临床表现的相关性。方法 对30例NSSI青少年(NSSI组)及29 名健康对照者(HC组)采集弥散张量成像(DTI),记录相关临床量表评分;比较组间白质网络拓扑属性的差异,分析差异脑区与临床量表评分的相关性。结果 组间全局网络属性差异均无统计学意义(P均>0.05)。相比HC组,NSSI组部分脑区局部网络度量指标差异有统计学意义,包括右侧额中回(眶部)、左侧额下回(三角部)、右侧脑岛、左侧内侧旁扣带回、右侧楔叶、右侧枕上回、右侧顶下缘角回、右侧角回及左侧丘脑节点效率(NE)降低而节点最短路径长度(NLP)增加(P均<0.05),左侧尾状核NLP增加(P<0.05),右侧额中回(眶部)、右侧脑岛、左侧内侧旁扣带回、左侧楔叶、右侧角回及左侧丘脑节点度中心性(DC)降低(P均<0.05),右侧额上回(眶部)DC增强(P<0.05)。NSSI组左侧额下回(三角部)、左侧内侧旁扣带回NLP及左侧丘脑DC与临床量表评分呈正相关(r:0.383~0.404,P均<0.05),左侧内侧旁扣带回NE和右侧额上回(眶部)DC与临床量表评分均呈负相关(r:-0.391、-0.425,P均<0.05)。结论 首发NSSI青少年腹外侧前额叶、边缘系统等情绪处理相关脑区存在白质局部网络拓扑属性破坏,且与临床表现相关。 |

Objective To observe the characteristics of topological metrics of brain white matter network in first-episode non-suicidal self-injury (NSSI) adolescents and correlations with clinical manifestations. Methods Data of diffusion tensor imaging (DTI) were prospectively acquired in 30 NSSI adolescents (NSSI group) and 29 healthy controls (HC group), and the clinical scales were recorded. The topological metrics of white matter network were compared between groups, and the correlations of topological metrics and clinical scale scores of NSSI group were analyzed. Results There was no significant difference of global network metrics between groups (all P>0.05), while significant differences of nodal network metrics were found in some brain regions of NSSI group compared with those of HC group, including decreased nodal efficiency (NE) but increased nodal shortest path (NLP) of right orbital part in middle frontal gyrus, left triangular part in inferior frontal gyrus, right insular, left median paracingulate gyrus, right cuneus, right superior occipital gyrus, right inferior parietal angular gyrus and right angular gyrus and left thalamus (all P<0.05), as well as increased NLP of left caudate nucleus (P<0.05) and decreased nodal degree centrality (DC) of right orbital part in middle frontal gyrus, right insular, left median paracingulate gyrus, left cuneus, right angular gyrus and left thalamus (all P<0.05), but increased DC of right orbital part in superior frontal gyrus (P<0.05). Clinical scale scores were positively correlated with NLP of the left triangular part in inferior frontal gyrus and the left median paracingulate gyrus, also DC of the left thalamus (r: 0.383—0.404, all P<0.05), while negatively correlated with NE of the left median paracingulate gyrus and DC of the right orbital part in superior frontal gyrus (r: -0.391, -0.425, both P<0.05) in NSSI group. Conclusion First-episode NSSI adolescents were found with destruction of white matter nodal network topological metrics related with brain regions of emotional processing, mainly involving ventrolateral prefrontal lobe and limbic system, being related to clinical symptoms. |
