      目的 对比观察经左、右侧肘正中静脉注射对比剂行冠状动脉CT血管成像(CCTA)的图像质量。方法 回顾性分析95例接受CCTA检查患者,其中40例经右侧(右臂组)、55例经左侧(左臂组)肘正中静脉以流率5 ml/s注射对比剂碘佛醇(350 mgI/ml)或碘普罗胺(370 mgI/ml)75 ml;于平静呼吸吸气末屏气行常规序列扫描,对比2组达触发阈值时间、触发浓度及CCTA图像质量。结果 2组CCTA图像质量均满足临床诊断需求(均≥2分)。组间达触发阈值时间、触发浓度、冠状动脉平均图像质量得分、主动脉根部图像噪声(SD主动脉),以及血管密度(CT主动脉)、校正后CT主动脉、冠状动脉血管密度(CT冠脉)、校正后CT冠脉、信噪比(SNR)及对比度噪声比(CNR)差异均无统计学意义(P均>0.05),年龄≥55岁患者上述CCTA图像质量主观及客观指标差异均无统计学意义(P均>0.05)。结论 经左或右侧肘正中静脉注射对比剂行CCTA所获图像质量相当。
英文摘要:
      Objective To compare the imaging quality of coronary CT angiography (CCTA) with contrast agent injection via the left or the right median cubital vein. Methods Data of 95 patients who underwent CCTA examination were retrospectively analyzed, including 40 cases (right arm group) underwent injection of contrast agent iodoformol (350 mgI/ml) or iopromide (370 mgI/ml) via the right and 55 cases (left arm group) via the left median cubital vein with flow rate of 5 ml/s and the total amount of 75 ml. Conventional sequence scanning was performed at the end of breathing and holding breath. The time to reach trigger threshold, trigger concentration and imaging quality of CCTA were compared between groups. Results The imaging quality of CCTA in both groups met the clinical diagnostic requirements (both ≥ 2 points). No significant difference of the time to reach the trigger threshold, trigger concentration, the mean coronary image quality score, image noise of the aortic root (SDaortic), vascular density (CTaortic), corrected CTaortic, coronary artery density (CTcoronary), corrected CTcoronary, signal-to-noise ratio (SNR) nor contrast noise ratio (CNR) was found between groups (all P>0.05), while no significant difference of the above subjective and objective indicators of CCTA imaging quality was found in patients aged ≥ 55 years between groups (all P>0.05). Conclusion The imaging quality of CCTA was similar with contrast agent injection via the left or the right median cubital vein.
