目的 以多模态MRI观察健康中老年人性激素、神经血管耦合及认知功能的相关性。方法 对43名认知能力正常的健康中老年志愿者采集MR高分辨3D T1WI、3D伪连续动脉自旋标记成像及静息态血氧水平依赖功能MRI (fMRI),并行标准化神经心理学评估;基于体素水平评价短程功能连接强度(sFCS)及脑血流量(CBF),计算CBF/sFCS,以评估神经血管耦合;采集外周静脉血,检测血清性激素水平。以相关性分析和中介分析观察性激素、神经血管耦合及认知功能的关系。结果 43名志愿者简易精神状态检查中位得分28.00(27.00,29.50)分,听觉语言学习测验(AVLT)-即刻评分为6.93±2.26,AVLT-延迟评分为7.10±3.97,AVLT-再认评分为13.00(12.00,14.00),词语流畅性测验评分为6.30(5.00,7.00),波士顿命名测试(BNT)评分为22.47±4.26;血清雌二醇44.77(47.44,131.84) pmol/L,促卵泡生成素(FSH)54.78(12.56,78.83) IU/L,黄体生成素(LH)19.72(7.57,30.13) IU/L,泌乳素(PRL)241.8(189.7,313.45) mIU/L,孕酮(Prog)0.46(0.47,0.83) nmol/L,睾酮(TES)0.54(0.30,7.34) nmol/L。双侧楔叶、双侧中央后回及左侧内侧额上回等脑区CBF/sFCS与血清FSH、LH、TES均呈正相关(r=0.599~0.817,P均<0.05);左侧内侧额上回CBF/sFCS与BNT呈正相关(r=0.311,P=0.042)。左侧内侧额上回CBF/sFCS部分介导TES对BNT的效应,其中介效应值0.479[95%CI(0.051,0.808)]。结论 健康中老年人部分性激素水平与神经血管耦合及认知功能之间存在一定相关性。 |

Objective To observe the correlations of sex hormones, neurovascular coupling and cognition function in healthy middle aged and elderly people with multi-modal MRI. Methods High-resolution 3D MR T1WI, 3D-pseudo continuous arterial spin labeling imaging and resting-state blood oxygen level dependent-functional MRI were acquired in 43 healthy middle aged or elderly volunteers with normal cognitive ability, and standardized neuropsychological assessments were performed. Short-functional connectivity strength (sFCS) and cerebral blood flow (CBF) were evaluated based on voxel-level, and CBF/sFCS were calculated to explore the neurovascular coupling. Peripheral venous blood was collected to examinate concentration of serum sex hormones. Correlation and mediation analysis were conducted to observe the relationships of sex hormones, neurovascular coupling and cognitive ability. Results The median mini-mental state examination score of 43 volunteers was 28.00 (27.00, 29.50). The auditory verbal learning test (AVLT)-immediate score was 6.93±2.26, AVLT-delayed score was 7.10±3.97, AVLT-recognized score was 13.00 (12.00, 14.00), verbal fluency test score was 6.30 (5.00, 7.00) and Boston naming test (BNT) score was 22.47±4.26. The estradiol was 44.77 (47.44, 131.84) pmol/L, follicle stimulating hormone (FSH) was 54.78 (12.56, 78.83) IU/L, luteinizing hormone (LH) was 19.72 (7.57, 30.13) IU/L, prolactin (PRL) was 241.8 (189.7, 313.45) mIU/L, progesterone (Prog) was 0.46 (0.47, 0.83) nmol/L, and testosterone (TES) was 0.54 (0.30, 7.34) nmol/L. CBF/sFCS of bilateral cuneus, bilateral posterior central gyrus and left medial superior frontal gyrus were positively correlated with serum FSH, LH and TES (r=0.599-0.817, all P<0.05), while of left medial superior frontal gyrus was positively correlated with BNT score (r=0.311, P=0.042). CBF/sFCS of left medial superior frontal gyrus partially mediated the effect of serum TES on BNT, with the mediation effect value of 0.479 (95%CI [0.051, 0.808]). Conclusion Some sex hormones, neurovascular coupling and cognitive ability had certain correlations in healthy middle-aged and elderly people. |
