目的 观察早孕期(孕11~13+6周)超声测量胎儿颅脑脉络丛(CP)及枕额径(OFD)对诊断开放性脊柱裂(OSB)的价值。方法 回顾性收集177胎早孕期超声未见明显异常、经系统产前超声筛查及出生后追踪证实无异常(正常组),以及25胎早孕期超声发现颅脑结构异常并经中期引产证实为OSB (OSB组)的胎儿,比较组间早孕期CP、OFD、脑干(BS)宽度、脑干至枕骨距离(BSOB)、CP/OFD及BS/BSOB值的差异,分析上述指标与胎儿头臀长(CRL)的相关性。结果 早孕期超声对正常组177胎胎儿均清晰显示CP及OFD,对其中150胎清晰显示、27胎无法显示BS及BSOB;对OSB组25胎均清晰显示CP及OFD,对其中18胎清晰显示、7胎无法显示BS及BSOB。相比正常组,OSB组胎儿BS宽度及CP均增大,BSOB及OFD均缩小,BS/BSOB及CP/OFD均增大(P均<0.001)。正常组胎儿BS宽度、BSOB、CP、OFD均与CRL呈正相关(r=0.592、0.794、0.565、0.772,P均<0.05),CP/OFD、BS/BSOB与CRL均呈负相关(r=-0.663、-0.276,P均<0.05)。OSB组胎儿校正后CRL与BS宽度、BSOB、CP、OFD均呈正相关(r=0.529、0.774、0.495、0.637,P均<0.05),与CP/OFD、BS/BSOB均呈负相关(r=-0.558、-0.231,P均<0.05)。结论 早孕期超声测量胎儿CP及OFD有助于尽早诊断胎儿OSB。 |

Objective To explore the value of fetal cerebral plexus (CP) and occipitofrontal diameter (OFD) measured with ultrasonography in early pregnancy (11-13+6 weeks) for diagnosing open spina bifida (OSB). Methods Totally 177 fetuses without obvious ultrasonic abnormalities during early pregnancy and confirmed by systematic prenatal ultrasound and screening after birth (normal group), as well as 25 fetuses with ultrasonic abnormal brain structures in early pregnancy and confirmed OSB (OSB group) by induction of labor enrolled. CP, OFD, brain-stem (BS) width, brain stem-occipital bone distance (BSOB), CP/OFD and BS/BSOB values in early pregnancy were compared between groups, and the correlations of the above indicators and crown-rump length (CRL) were analyzed. Results Early pregnancy ultrasound clearly showed CP and OFD in all 177 fetuses in normal group, BS and BSOB were clearly displayed in 150 fetuses, but not in 27 fetuses. In OSB group, CP and OFD could be clearly found in all 25 fetuses, BS and BSOB were clearly showed in 18 fetuses, but not in 7 fetuses. Compared with normal group, in OSB group, fetal BS width and CP increased, BSOB and OFD decreased, while both BS/BSOB and CP/OFD increased (all P<0.001). In normal group, fetal BS width, BSOB, CP and OFD were positively correlated with CRL (r=0.592, 0.794, 0.565, 0.772, all P<0.05), while CP/OFD, BS/BSOB and CRL were negatively correlated CRL (r=-0.663, -0.276, both P<0.05). In OSB group, fetal CRL after correction were positive correlated with BS width, BSOB, CP and OFD (r=0.529, 0.774, 0.495, 0.637, all P<0.05), but negatively correlated with CP/OFD and BS/BSOB (r=-0.558, -0.231, both P<0.05). Conclusion Fetal CP and OFD measured with fetal ultrasound in early pregnancy was helpful for early diagnosis of OSB. |
