      目的 观察肺孤立性结节样腺癌PET/CT表现与丙酮酸激酶M2(PKM2)、硒结合蛋白-1(SBP-1)表达的相关性。方法 以101例肺孤立性结节样腺癌为观察组,以68例肺良性肿瘤为对照组;比较2组病灶PKM2、SBP-1表达及PET/CT最大标准摄取值(SUVmax)、峰值标准摄取值(SUVpeak)和肿瘤代谢体积(MTV),分析观察组病灶PET/CT表现与PKM2、SBP-1表达的关系。结果 观察组病灶PKM2高表达率高于(P<0.001)、SBP-1高表达率低于对照组(P<0.001);观察组病灶SUVmax、SUVpeak及MTV均高于对照组(P均<0.05)。观察组内PKM2高表达结节呈分叶状、多边形/不规则形、胸膜凹陷征占比及其SUVmax、SUVpeak和MTV均高于PKM2低表达结节(P均<0.05);SBP-1高表达结节呈分叶状、胸膜凹陷征占比及其SUVmax、SUVpeak和MTV均低于SBP-1低表达结节(P均<0.05)。观察组病灶呈分叶状、出现胸膜凹陷征、SUVmax、SUVpeak及MTV均与其PKM2表达呈正相关、与SBP-1表达呈负相关(P均<0.05)。结论 肺孤立性结节样腺癌PET/CT表现与其PKM2及SBP-1表达相关。
英文摘要:
      Objective To observe the correlations of PET/CT findings, expression of pyruvate kinase M2 (PKM2) and selenium binding protein 1 (SBP-1) of pulmonary solitary nodular adenocarcinoma. Methods A total of 101 patients with pulmonary solitary nodular adenocarcinoma (study group) and 68 patients with benign lung tumors (control group) were enrolled. The expression of PKM2 and SBP-1, the maximum standard uptake value (SUVmax), peak standard uptake value (SUVpeak) and metabolic tumor volume (MTV) of lesions were compared between groups. The correlations of PET/CT manifestations, the expression of PKM2 and SBP-1 of lesions in study group were analyzed. Results High expression rate of PKM2 of lesions in study group was higher (P<0.001), while of SBP-1 was lower than those in control group (P<0.001). SUVmax, SUVpeak and MTV of lesions in study group were all higher than those in control group (all P<0.05). In study group, the percentage of lobar sign, polygonal/irregular shape and pleural depression sign, as well as SUVmax, SUVpeak and MTV of nodules with high PKM2 expression were higher than those of nodules with low PKM2 expression (all P<0.05), while the percentage of lobar sign and pleural depression sign, as well as SUVmax, SUVpeak and MTV of nodules with high SBP-1 expression were lower than those of nodules with low SBP-1 expression (all P<0.05). The lobar sign, pleural depression sign, SUVmax, SUVpeak and MTV of lesions in study group were positively correlated with PKM2 expression but negatively correlated with SBP-1 expression (all P<0.05). Conclusion PET/CT findings of pulmonary solitary nodular adenocarcinoma were correlated with the expression of PKM2 and SBP-1.
