      目的 观察经腹超声测量直肠横径(RD)用于诊断儿童膀胱直肠功能障碍(BBD)的价值。方法 对158例临床诊断BBD患儿(观察组)及142名接受健康体检的正常儿童(对照组)行经腹超声并测量RD,比较其组间差异;以临床诊断为标准,绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估经腹超声测量RD诊断儿童BBD的效能。结果 观察组RD[(34.19±6.51)mm]大于对照组[(23.58±8.70)mm,t=12.028,P<0.001]。以RD诊断儿童BBD的AUC为0.83,以25.53 mm为最佳临界值,诊断敏感度为92.41%,特异度为61.97%,阳性预测值为73.00%,阴性预测值为88.00%。结论 经腹超声测量RD有助于诊断儿童BBD。
英文摘要:
      Objective To explore the value of transverse rectal diameter (RD) measured with transabdominal ultrasound for diagnosing bladder and bowel dysfunction (BBD) in children.Methods Transabdominal ultrasound was performed in 158 children with clinically diagnosed BBD (observation group) and 142 healthy children (control group) to measure RD. Then RD was compared between groups. Receiver operating characteristic (ROC) curve was drawn according to the clinical diagnosis, and the area under the curve (AUC) was calculated to evaluate the efficacy of RD for diagnosing BBD in children.Results RD of observation group ([34.19±6.51]mm) was higher than that of control group ([23.58±8.70] mm, t=12.028, P<0.001). AUC of RD for diagnosing BBD in children was 0.83. Taken 25.53 mm as the optimal critical value, the sensitivity, specificity, positive predictive value and negative predictive value of RD for diagnosing BBD in children was 92.41%, 61.97%, 73.00% and 88.00%, respectively.Conclusion RD measured with transabdominal ultrasound was helpful for diagnosing BBD in children.
