目的 观察3.0T MR T1ρ及T2 mapping评估兔股骨内侧髁关节软骨退变的价值。方法 将40只兔随机分为2周组、4周组、6周组及对照组各10只。对2周组、4周组及6周组兔建立右后肢膝骨关节炎模型并分别制动2、4及6周,对照组不予处理;行T1ρ及T2 mapping成像。处死动物后取股骨内侧髁关节软骨病变最严重区域行病理切片及染色;根据国际骨关节炎研究学会(OARSI)分级标准分为正常组、退变早期组及退变中晚期组,测定Ⅱ型胶原、蛋白多糖、β-Catenin及基质金属蛋白酶(MMP)-13,观察T1ρ及T2 mapping评估其关节软骨退变的价值。结果 最终对37只兔造模成功,正常组8只、早期退变组20只、中晚期退变组9只;3组股骨内侧髁关节软骨T1ρ值及T2值差异均有统计学意义(P均<0.05),两两比较差异亦有统计学意义(P均<0.05)。T1ρ值及T2值与OARSI分级(r=0.72、0.73,P均<0.01)、MMP-13表达(r=0.84、0.59,P均<0.01)及β-Catenin表达(r=0.76、0.66,P均<0.01)均呈正相关,与Ⅱ型胶原含量(r=-0.70、-0.61,P均<0.01)及蛋白多糖含量(r=-0.82、-0.57,P均<0.01)均呈负相关。对正常组及退变早期组共28只兔行logistic回归分析,T1ρ值联合T2值鉴别兔正常与早期右后肢股骨内侧髁关节软骨退变的曲线下面积为0.84,敏感度及特异度分别为90.40%及75.26%。结论 3.0T MR T1ρ及T2 mapping可定量评估兔股骨内侧髁关节软骨退变,用于判断其早期退变较具价值。 |

Objective To observe the value of 3.0T MR T1ρ and T2 mapping for evaluating articular cartilage degeneration of medial femoral condyle in rabbits. Methods Forty rabbits were randomly divided into 2-week group, 4-week group, 6-week group and control group (each n=10). The knee osteoarthritis models of the right hind limb were established in 2-week, 4-week and 6-week group with braking time of 2, 4 and 6 weeks, respectively. T1ρ and T2 mapping of the right hind knee joint were performed. The rabbits were then sacrificed, the most serious areas of articular cartilage lesion of medial femoral condyle were taken for pathological section and staining. The samples were divided into normal group, early degeneration group, middle and late degeneration group according to Osteoarthritis Research Society International (OARSI), and type Ⅱ collagen, proteoglycan, β-Catenin and matrix metalloproteinase (MMP)-13 were detected. The value of T1ρ and T2 mapping for evaluating articular cartilage degeneration were analyzed. Results The models were successfully established in 37 rabbits, including 8 in normal group, 20 in early degeneration group and 9 in middle and late degeneration group. T1ρ and T2 values of articular cartilage of medial femoral condyle of the right hind limb were significant different among 3 groups (all P<0.05), as well as between each 2 groups (all P<0.05). T1ρ and T2 values were positively correlated with OARSI grade (r=0.72, 0.73, both P<0.01), MMP-13 (r=0.84, 0.59, both P<0.01) and β-Catenin expression (r=0.76, 0.66, both P<0.01), while negatively correlated with type Ⅱ collagen (r=-0.70, -0.61, both P<0.01) and proteoglycan (r=-0.82, -0.57, both P<0.01). Logistic regression analysis was performed based on 28 rabbits of normal group and early degeneration group, and the results showed that the area under the curve of T1ρ value combined with T2 value for differentiating articular cartilage of the medial femoral condyle of the right hind limb being normal or in early degeneration stage was 0.84, with sensitivity of 90.40% and specificity of 75.26%, respectively. Conclusion 3.0T MR T1ρ and T2 mapping could quantitatively evaluate articular cartilage degeneration of medial femoral condyle in rabbits, and were valuable for judging early degeneration. |
