目的 评价光谱CT虚拟单能量图像(VMI)优化胸部增强CT显示肺动脉的价值,观察可否以碘密度预测其最佳单能级水平。方法 分析40例胸部增强CT显示肺动脉强化不佳(肺动脉干CT值<200 HU)患者资料,分别以常规120 kVp混合能量图像(常规图像)和40~70 keV VMI(间隔10 keV)测量并计算肺动脉干、左肺动脉、右肺动脉、左上肺动脉、左下肺动脉、右上肺动脉及右下肺动脉的CT值、噪声(SD)、信噪比(SNR)及对比噪声比(CNR),并以5分法评价图像质量。以肺动脉CT值>200 HU且主观评分 ≥ 3分选择最佳优化VMI,将该能级下VMI的主观、客观指标与常规图像进行比较;以线性回归模型评估平均碘密度与最佳单能级的相关性。结果 于40~70 keV VMI所测肺动脉CT值、SNR、CNR及主观评分均随能级降低而递增,SD值则递减,与常规图像相比差异多有统计学意义(P<0.05)。40例中,36例(36/40, 90.00%)图像经优化达到诊断要求;最佳单能级水平下,优化图像的CT值、SNR和CNR分别较常规图像提高59.61%、76.51%和124.03%而SD值降低12.77%,主观评分提高148.15%。线性回归模型显示,平均碘密度与最佳单能级呈正相关 (R2=0.80)。结论 光谱CT VMI可优化胸部增强CT显示肺动脉质量;根据碘密度可预测最佳单能级水平。 |

Objective To observe the value of virtual monoenergetic image (VMI) of spectral chest CT for optimizing displaying of pulmonary arteries in enhanced chest CT, and to explore whether iodine density can be used to predict the optimal monoenergetic level. Methods Data of enhanced chest CT of 40 patients with poor pulmonary arteries enhancement (CT value of pulmonary trunk <200 HU) were retrospectively analyzed. CT value, noise value (SD), signal-to-noise ratio (SNR) and contrast-noise ratio (CNR) of pulmonary trunk, left pulmonary artery, right pulmonary artery, left superior pulmonary artery, left inferior pulmonary artery, right superior pulmonary artery and right inferior pulmonary artery were measured and calculated on conventional 120 kVp mixed energy images (conventional images) and VMI of 40-70 keV (10 keV intervals), and imaging qualities were evaluated using 5-point method. The optimal VMI was selected with the pulmonary artery CT value >200 HU and subjective score ≥ 3, the subjective and objective parameters of VMI were compared with those of conventional images at this monoenergetic level. Linear regression model was used to evaluate the correlation between average iodine density and optimal monoenergetic level. Results CT value, SNR, CNR and subjective score of pulmonary arteries from 40 to 70 keV VMI increased but SD value decreased with the decrease of monoenergetic levels, most being different with those of conventional images (P<0.05). The displaying of pulmonary arteries were optimized and met the diagnostic requirements in 36 cases (36/40, 90.00%). At the optimal monoenergetic level, CT value, SNR and CNR of the images increased by 59.61%, 76.51% and 124.03% compared with those of conventional images, respectively, SD value reduced by 12.77%, while the subjective score increased by 148.15%. The linear regression model showed that the average iodine density was positively correlated with the optimal monoenergetic level of VMI (R2=0.80). Conclusion VMI of spectral CT could optimize the displaying of pulmonary arteries in enhanced chest CT.Iodine density could be used to predict the optimal monoenergetic level of VMI. |
