目的 观察基于射频信号全息血管硬度分析(R-QVS)和动态向量血流成像(V Flow)评估青年男性H型高血压(HTN)患者颈动脉结构及功能改变的价值。方法 收集134例首诊青年男性原发性HTN患者,依据血浆同型半胱氨酸(Hcy)水平及血压分为1级H型HTN(Hcy ≥ 15 μmol/L且血压为140~159/90~99 mmHg,HH1组,n=33)、2级H型HTN(Hcy ≥ 15 μmol/L且血压 ≥ 160/100 mmHg,HH2组,n=33)、1级单纯型HTN(Hcy<15 μmol/L且血压为140~159/90~99 mmHg,SH1组,n=36)及2级单纯型HTN(Hcy<15 μmol/L且血压 ≥ 160/100 mmHg,SH2组,n=32);选取同期60名男性健康体检者,分为正常血压及正常Hcy(NH1组,n=30)和正常血压高Hcy(NH2组,n=30)。以全息血管内-中膜厚度(RIMT)技术测量颈总动脉内-中膜厚度(IMT),以R-QVS测量收缩期颈总动脉远段管径(Diam)、血管壁位移(Dist)、硬度指数(HC)和脉搏波传导速度(PWV),以V Flow模式测量该处血管后壁最大壁面剪切应力(WSSmax)和最大瞬时流速(Vmax)。比较6组超声参数差异,绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评价其评估青年男性H型HTN患者颈动脉结构及功能改变的效能。结果 6组颈总动脉IMT、Diam、Dist、HC、PWV、WSSmax及Vmax差异均有统计学意义(P均<0.05)。以颈总动脉超声参数IMT、Diam、Dist、HC、PWV、WSSmax及Vmax评估青年男性H型HTN患者颈动脉结构及功能改变的AUC分别为0.81、0.82、0.30、0.82、0.88、0.20及0.23。结论 R-QVS和V Flow技术有助于评估青年男性H型HTN患者颈动脉结构及功能改变。 |

Objective To observe the value of radio frequency-data based quantitative analysis on vessel stiffness (R-QVS) and high frame rate vector flow imaging (V Flow) for evaluating carotid artery structure and function in young male patients with H-type hypertension (HTN). Methods Totally 134 young males with first diagnosed HTN were enrolled and divided into grade 1 H-type HTN (Hcy ≥ 15 μmol/L and blood pressure 140-159/90-99 mmHg, HH1 group, n=33), grade 2 H-type HTN (Hcy ≥ 15 μmol/L and blood pressure ≥ 160/100 mmHg, HH2 group, n=33), grade 1 simple HTN (Hcy<15 μmol/L and blood pressure 140-159/90-99 mmHg, SH1 group, n=36) and grade 2 simple HTN (Hcy<15 μmol/L and blood pressure ≥ 160/100 mmHg, SH2 group, n=32) groups according to plasma homocysteinemia (Hcy) level and blood pressure. Meanwhile, 60 male healthy adults with matched general information were elected and divided into normal blood pressure and Hcy group (NH1 group, n=30) and normal blood pressure with high Hcy group (NH2 group, n=30). Intima-media thickness (IMT) of the common carotid artery was measured using real-time IMT (RIMT) technique, while distal common carotid artery vessel diameter (Diam), displacement (Dist), hardness coefficient (HC) and pulse wave velocity (PWV) were measured with R-QVS. The maximum wall shear stress (WSSmax) and the maximum instantaneous velocity (Vmax) of the posterior wall of that site were measured using V Flow. Then the ultrasonic parameters were compared among 6 groups. Receiver operating characteristic (ROC) curve was drawn, and the area under the curve (AUC) was calculated to analyze the efficacy of each parameter for evaluating carotid artery structure and function in young male patients with NTH. Results Significant differences of common carotid artery IMT, Diam, Dist, HC, PWV, WSSmax and Vmax were found among 6 groups (all P<0.05). AUC of IMT, Diam, Dist, HC, PWV, WSSmax and Vmax for evaluating carotid artery structure and function in young male patients with NTH was 0.81, 0.82, 0.30, 0.82, 0.88, 0.20 and 0.23, respectively. Conclusion R-QVS and V Flow technique were beneficial for evaluating carotid artery structure and function in young male patients with NTH. |
