      目的 观察自研心腔内超声心动图(ICE)成像系统用于大动物实验的性能及其安全性。方法 以超声体模测试自研ICE系统的成像分辨力。对4只约克夏猪行活体ICE成像,评估成像视野、对心脏结构的分辨力,以及ICE导管血管内通过性、操作便利性及其安全性,并与临床ICE系统进行对比。对其中2只猪行ICE引导下房间隔穿刺,观察其可行性和安全性。结果 自研ICE成像系统心腔内最大探测深度 ≥ 60 mm,轴向与侧向分辨力均 ≤ 1.0 mm,横向及纵向几何位置精度分别为 ≤ 10%及 ≤ 5%,图像几何畸变 ≤ 10%。活体ICE成像中,自研ICE导管均于超声引导下顺利经股静脉进入右心房,通过尾端控制手柄调整导管头端角度均可显示目标切面,心腔结构清晰可辨,无明显噪声,成像质量与临床ICE系统接近。在自研ICE引导下对2只猪均一次穿刺房间隔成功,未出现急性手术并发症。操作后4只实验猪均健康存活,未见介入相关血管、器官及系统损伤。结论 自研ICE成像系统性能稳定、安全,可用于更大规模临床前大动物实验。

      Objective To observe the performances and safety of self-developed intracardiac echocardiography (ICE) system in large animals. Methods An ultrasonic phantom was employed to measure imaging resolution of the self-developed ICE system,while its capability of displaying cardiac structure and passing through blood vessels, relative complications and safety were explored in vivo using 4 Yorkshire hogs and compared with those of clinical used ICE system. Atrial septal puncture was performed under the guidance of self-developed ICE system in 2 Yorkshire hogs to assess its feasibility and safety. Results The maximum detection depth of cardiac cavity of self-developed ICE imaging system was ≥ 60 mm, with both axial and lateral resolution ≤ 1.0 mm, the lateral and longitudinal geometric position accuracy ≤ 10% and ≤ 5%, respectively, and the image geometric distortion ≤ 10%. The catheter could be easily inserted into right atrial cavity of hogs through the right femoral vein under the guidance of ultrasound. For in vivo imaging, images of target sectors acquired easily, and the cardiac structures could be clearly identified with the help of handle. The imaging performances of the self-developed ICE imaging system were comparable with those of the clinical used ICE system. Atrial septal punctures were performed successfully in 2 hogs without acute operation complications. All 4 hogs survived healthily after experiments, without vessel, organ nor system damage related to intervention. Conclusion The self-developed ICE system had stable and safe performances, could be used in large animals.
