      目的 观察乳腺局灶异常的平扫锥形束乳腺CT (CBBCT)表现及其诊断价值。方法 建立含纯水、纯脂、肌肉及钙化模块的牛乳腺体外模型,比较其平扫CBBCT、平扫传统螺旋CT及乳腺X线摄影(MG)表现。收集65例乳腺局灶异常患者,观察囊性病灶的囊变区、实性灶、含脂灶及钙化灶平扫CBBCT表现;分析平扫CBBCT的诊断价值。结果 平扫CBBCT示牛乳腺、纯水、纯脂、肌肉及钙化模块的CT值分别为107.1、114.5、-48.8、149.8及459.8 HU;螺旋CT平扫纵隔窗示牛乳腺、纯水、纯脂及肌肉的CT值分别为37.2、9.4、-114.9及46.4 HU,而钙化显示不清;MG示纯水、纯脂、肌肉及钙化相对于周围牛乳腺分别呈等密度、明显低密度、稍高密度及明显高密度。65例患者中,平扫CBBCT示乳腺囊性灶的囊变区均呈等密度,实性灶呈等或稍高密度,含脂灶均呈低密度,钙化灶均呈高密度;乳腺囊性灶的囊变区、实性灶、含脂灶及钙化灶CT值差异有统计学意义(P<0.01);两两比较,除囊性灶的囊变区与实性灶外,其余CT值差异均有统计学意义(P均<0.05)。乳腺含脂灶及钙化灶与周围腺体的CT值差异均有统计学意义(P均<0.05)。结论 乳腺局灶异常的平扫CBBCT表现与MG较为接近,而与传统螺旋CT差异较大;平扫CBBCT所示CT值有助于判断其内成分。

      Objective To observe the plain cone beam breast CT (CBBCT) findings and diagnostic value of breast focal abnormalities. Methods In vitro models of bovine mammary gland containing pure water, pure fat, muscle and calcification were established, and the findings of plain CBBCT, plain traditional spiral CT and mammography (MG) were compared. Then 65 patients with breast focal abnormalities were enrolled, and the plain CBBCT findings of cystic areas of cystic abnormalities, solid abnormalities, fatty abnormalities and calcifications of breast were observed. The value of plain CBBCT for diagnosing breast focal abnormalities were analyzed. Results Plain CBBCT of in vitro models showed that the CT value of bovine mammary gland, pure water, pure fat, muscle and calcification was 107.1, 114.5, -48.8, 149.8 and 459.8 HU, respectively. Plain traditional spiral CT mediastinal window images showed the CT value of bovine mammary gland, pure water, pure fat and muscle was 37.2, 9.4, -114.9 and 46.4 HU, respectively, but calcifications were not clearly showed. On MG images, pure water, pure fat, muscle and calcification presented as equal density, obviously low density, slightly high density and significantly high density compared with the surrounding bovine mammary gland, respectively. Plain CBBCT of breast focal abnormalities in 65 patients showed that cystic areas of cystic abnormalities presented as equal densities, solid ones presented as equal or slightly high densities, fatty ones presented as low densities, and calcifications presented as high densities. There were significant differences among CT values of cystic, solid, fatty abnormalities and calcifications of patients' breasts (P<0.01). Except for CT values between cystic and solid abnormalities, there were significant differences of CT values between each 2 kinds of abnormalities (all P<0.05). There were significant differences of CT values between fatty abnormalities and calcifications of breast and glands around the abnormalities (both P<0.05). Conclusion Plain CBBCT findings of different breast focal abnormalities were close to those of MG, but different from those of traditional spiral CT. CT value showed on plain CBBCT could assist to judge the internal components of breast focal abnormalities.
