      目的 观察18F-FDG PET/CT脑显像联合韦氏儿童智力量表第四版(WISC-Ⅳ)评估癫痫患儿认知功能的价值。方法 回顾性分析85例接受18F-FDG PET/CT脑显像及WISC-Ⅳ检测的癫痫患儿资料;根据PET/CT所示脑代谢异常范围将其分为单脑叶组(n=40)和多脑叶组(n=45),并根据PET/CT示脑代谢异常灶是否位于颞叶进一步将单脑叶组患儿分为颞叶亚组(n=16)和非颞叶亚组(n=24)。比较组间及亚组间WISC-Ⅳ各项合成分数,观察病程及起病年龄与WISC-Ⅳ各项合成分数的相关性,以及单脑叶组不对称指数(AI)与年龄、起病年龄、病程及WISC-Ⅳ各项合成分数的相关性;分析18F-FDG PET/CT联合WISC-Ⅳ评估癫痫患儿认知功能的价值。结果 单脑叶组患儿知觉推理指数、工作记忆指数及全量表智商(IQ)均显著高于多脑叶组(P均<0.05)。颞叶亚组患儿言语理解指数低于非颞叶亚组(P<0.05)。癫痫患儿病程与言语理解指数和全量表IQ均呈负相关(r=-0.404,P<0.001;r=-0.289,P=0.007),而与知觉推理指数、工作记忆指数及加工速度指数均无明显相关性(P均>0.05);起病年龄与WISC-Ⅳ各项合成分数均无明显相关性(P均>0.05)。单脑叶组|AI|与患儿年龄、起病年龄、病程及WISC-Ⅳ各项合成分数均无明显相关性(P均>0.05)。结论 18F-FDG PET/CT脑显像联合WISC-Ⅳ有助于评估癫痫患儿认知功能。

      Objective To observe the value of 18F-FDG PET/CT brain imaging combined with Wechsler intelligence scale for children-fourth edition (WISC-Ⅳ) for evaluating cognitive function in children with epilepsy. Methods Data of 85 children with epilepsy who underwent 18F-FDG PET/CT brain imaging and WISC-Ⅳ were retrospectively analyzed. The children were then divided into single lobe group (n=40) and multi lobe group (n=45) according to the abnormal ranges of brain metabolism shown on PET/CT, and those in single lobe group were further divided into temporal lobe subgroup (n=16) and non-temporal lobe subgroup (n=24) according to the abnormal focus of brain metabolism located in temporal lobe or not. WISC-Ⅳ composite scores were compared between groups and subgroups, and the correlations of WISC-Ⅳ composite scores and the course of disease, also the age of onset were observed, as well as asymmetry index (AI) and age, the age of onset, the course of disease, WISC-Ⅳ composite scores of single lobe group. The value of 18F-FDG PET/CT combined with WISC-Ⅳ for evaluating cognitive function in children with epilepsy was analyzed. Results The perceptual reasoning index, working memory index and full-scale intelligence quotient (IQ) of children in single lobe group were significantly higher than those in multi lobe group (all P<0.05), and the verbal comprehension index of children in temporal lobe subgroup was lower than that in non-temporal lobe subgroup (P<0.05). The course of disease was negatively correlated with verbal comprehension index and full-scale IQ (r=-0.404, P<0.001; r=-0.289, P=0.007), but not significantly correlated with perceptual reasoning index, working memory index nor processing speed index (all P>0.05). No significant correlation was found between|AI|and the children's age, the age of onset, the course of disease nor WISC-Ⅳ composite scores in single lobe group (all P>0.05). Conclusion 18F-FDG PET/CT brain imaging combined with WISC-Ⅳ was helpful for evaluating cognitive function in children with epilepsy.
