目的 观察经会阴盆底超声用于督导盆底功能障碍(PFD)患者盆底肌训练(PFMT)并定量评估其疗效的价值。方法 前瞻性纳入158例PFD患者,并将其随机分为研究组(n=69)和对照组(n=89),嘱患者完成12周PFMT,且对研究组额外增加超声督导、健康宣教及跟踪反馈;采用盆底超声评估患者PFMT前、后盆底功能,包括静息态及最大瓦尔萨尔瓦动作下膀胱颈位置及肛提肌裂孔面积,并分析其价值。结果 PFMT前、后,静息态及最大瓦尔萨尔瓦动作下2组膀胱颈位置及肛提肌裂孔面积差异均有统计学意义(P均<0.05);且PFMT后,2组膀胱颈位置及肛提肌裂孔面积差异亦有统计学意义(P均<0.05)。研究组患者PFMT依从性优于对照组(P<0.05)。结论 经会阴盆底超声可用于督导PFD患者PFMT并定量评估其疗效。 |

Objective To explore the value of transperineal pelvic floor ultrasound for supervising pelvic floor muscle training (PFMT) and quantifying its curative effect in patients with pelvic floor dysfunction (PFD).Methods A total of 158 PFD patients were prospectively enrolled and randomly divided into study group (n=69) and control group (n=89). The patients were all instructed to complete PFMT for 12 weeks, and ultrasound supervision, health education and follow-up feedback were added to study group. Pelvic floor ultrasound was used to evaluate pelvic floor function before and after PFMT, including the position of bladder neck, the area of levator ani hiatus under resting-state and the maximum Valsalva maneuver, and the values of the above indexes were observed.Results There were significant differences of the position of bladder neck and the area of levator ani hiatus under resting-state and the maximum Valsalva maneuver of 2 groups before and after PFMT (all P<0.05), as well as the position of bladder neck and the area of levator ani hiatus after PFMT between 2 groups (all P<0.05). The compliance of PFMT in study group was better than that in control group (P<0.05).Conclusion Transperineal pelvic floor ultrasound could be used for supervising PFMT quantifying its curative effect in PFD patients. |
