目的 观察iworks智能工作流用于孕中期超声检查流程优化及质量控制的价值。方法 纳入2 658胎在20家产前诊断中心接受孕中期超声检查胎儿,根据检查方法将其分别纳入iworks组(n=1 394)和非iworks组(n=1 264)。对iworks组胎儿,医师根据iworks要求进行检查并存储42个标准切面;对非iworks组胎儿则由经统一培训后的医师自主存储并核对标准切面。比较2组检查次数、总检查时间及标准切面的完整性,分析影响iworks总检查时间的因素。结果 iworks组均存储42个标准切面;非iworks组80.30%(1 015/1 264)完整存储全部标准切面,126胎缺少1个切面、68胎缺少2个切面、26胎缺少3个切面、29胎缺少3个以上切面。2组超声检查次数差异无统计学意义(P>0.05),但非iworks组平均总检查时间短于iworks组(P<0.001);2组首检胎儿头部及脊柱位置差异均有统计学意义(P均<0.05)。孕妇年龄、孕龄及医师操作软件天数均为iworks组总检查时间的影响因素(P均<0.05),孕妇体质量指数、胎儿头部及脊柱位置则与总检查时间无明显相关性(P均>0.05)。结论 iworks智能工作流可完整留存胎儿标准切面,优化孕中期超声检查流程及质量控制。 |

Objective To observe the impact of the iworks intelligent protocol on the workflow optimization and quality control of second trimester of pregnancy ultrasound examination. Methods Totally 2 658 fetuses that underwent second trimester of pregnancy ultrasound examination in 20 obstetric ultrasound units were enrolled and divided into iworks group (n=1 394) and non-iworks group (n=1 264). For fetuses in iworks group, doctors were supposed to store 42 fetal standard planes with adherence to the intelligent protocol, while standard views were voluntarily recorded and evaluated by doctors after training program in non-iworks group. The examination time, total examination time and integrity of fetal standard views were compared between groups, while the impact factors potentially related with the examination time of iworks group were analyzed. Results All ultrasound examinations in iworks group included 42 standard views, while total display of standard views were achieved in 80.30% (1 015/1 264) fetuses in non-iworks group, but with absence of 1 plane in 126 fetuses, 2 planes in 68, 3 planes in 26 and or more than 3 planes in 29 fetuses. The examination time was not significantly different between groups (P>0.05), but the total examination time of non-iworks group was significantly shorter than that of iworks group (P<0.001). The position of fetal head and spine during the first examination were significantly different between groups (both P<0.05). The examination time in iworks group was significantly related with maternal age, gestational age and the days that doctors operated the software (all P<0.05), whereas the maternal body mass index and position of fetal head and spine showed no significant correlation with examination time (all P>0.05). Conclusion The iworks intelligent protocol could completely retain the standard section of fetus, optimize the second trimester of pregnancy ultrasound examination process and carry out quality control. |
