      目的 采用Meta分析方法评价肺部超声诊断急性心力衰竭(AHF)的价值。方法 检索PubMed、Cochrane Library、Web of Science、EMbase、中国知网、万方医学网和维普数据库中关于肺部超声诊断AHF的相关文献,检索时间自建库至2021年5月;根据标准筛选文献,提取相关资料。采用Stata 16.0软件分析肺部超声对AHF的诊断效能。结果 共纳入13篇文献、2 484例患者,包括1 277例AHF 和1 207例非AHF。汇总分析结果显示,肺部超声诊断AHF的敏感度为0.89,特异度为0.90,阳性似然比8.57,阴性似然比为0.12,综合受试者工作特征(SROC)曲线下面积为0.94。肺部超声诊断AHF验前概率20%,验后概率升至68%。结论 肺部超声对诊断AHF具有较高价值。

      Objective To explore the diagnostic value of lung ultrasound for acute heart failure (AHF) with meta-analysis. Methods PubMed, Cochrane Library, Web of Science, EMbase, CNKI, Wanfang Med Online and VIP databases were searched to obtain relevant published literature on lung ultrasound in diagnosis of AHF before May 2021. The literature were screened according to the criteria, and the relevant data were extracted. Stata 16.0 software was used to analyze the efficacy of lung ultrasound for diagnosing AHF. Results Thirteen literatures were enrolled, including 2 484 patients (1 277 cases of AHF and 1 207 cases of non-AHF). Meta-analysis results showed that the summary sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and the area under the summary receiver operating characteristic (SROC) curve of lung ultrasound for diagnosing AHF were 0.89 (95%CI), 0.90 (95%CI), 8.57 (95%CI), 0.12 (95%CI) and 0.94 (95%CI), respectively. The posterior probability of pulmonary ultrasound in diagnosis of AHF increased to 68% from the prior probability of 20%. Conclusion Lung ultrasound had high value for diagnosis of AHF.
