目的 观察99Tcm-MDP SPECT/CT诊断布鲁氏菌病脊柱炎(BS)的价值。方法 纳入20例BS患者,包括急性期7例、慢性期13例,均接受99Tcm-MDP SPECT全身骨显像及局部SPECT/CT显像;观察病变椎体、软骨终板及椎间隙,测量受累椎体及相邻未受累椎体的最大放射性计数,评价SPECT/CT诊断BS的价值。结果 CT检出24个(61.54%,24/39)病变椎体,其中腰椎21个、胸椎3个,主要表现为椎体上下缘骨质破坏,边缘呈花边样改变,伴增生、硬化,部分相邻椎间隙狭窄。SPECT及SPECT/CT均检出39个(100%,39/39)受累椎体,其中腰椎25个、胸椎14个,主要表现为异常放射性浓聚。急性期、慢性期BS病灶及相邻未受累椎体的最大放射性计数分别为11 339.56±1 384.23、7 746.96±1 087.57及5 179.12±744.39,两两比较差异均有统计学意义(P均<0.01)。结论 99Tcm-MDP SPECT/CT可同时显示BS病灶最大放射性计数及骨组织结构改变,有助于早期诊断BS。 |

Objective To observe the value of 99Tcm-MDP SPECT/CT for diagnosing brucellosis spondylitis (BS). Methods Totally 20 patients with BS were enrolled, including 7 cases in acute stage and 13 cases in chronic stage. All patients underwent 99Tcm-MDP SPECT whole-body bone imaging and local SPECT/CT imaging. The involved vertebral body, cartilage endplate and intervertebral space were observed, and the maximum radioactive count of involved vertebral body and adjacent non-involved vertebral body were measured. The value of SPECT/CT for diagnosing BS was analyzed. Results CT detected 24 involved vertebrae (61.54%, 24/39), including 21 lumbar vertebrae and 3 thoracic vertebrae, mainly manifested as bone destruction at the upper and lower edges of the vertebral body and "lace like" changes at the edges, with hyperplasia, sclerosis and stenosis of some adjacent intervertebral spaces. SPECT and SPECT/CT detected 39 involved vertebrae (100%, 39/39), including 25 lumbar vertebrae and 14 thoracic vertebrae, mainly manifested as abnormal concentration of radioactivity in vertebral body. The maximum radioactive count in acute and chronic stage BS lesions was 11 339.56±1 384.23 and 7 746.96±1 087.57, respectively, while of non-involved vertebral body was 5 179.12±744.39, and there were significant differences between each 2 groups (all P<0.01). Conclusion 99Tcm-MDP SPECT/CT could simultaneously display the maximum radioactive count and changes of bone structure in BS lesions, hence being helpful to early diagnosis of BS. |
