      目的 以定量磁化率成像(QSM)观察复发缓解型多发性硬化(RRMS)及视神经脊髓炎谱系疾病(NMOSD)患者深部灰质铁含量差异及其与运动及认知障碍的关系。方法 针对35例RRMS (RRMS组)、30例NMOSD (NMOSD组)患者及30名健康人(对照组)基于颅脑QSM获取各灰质核团定量磁化率值(QSV),比较组间各QSV及功能评分差异,以相关分析评估灰质核团QSV异常与患者运动及认知障碍之间的关系。结果 3组间蒙特利尔认知评估(MoCA)、数字广度测验(DST)、符号数字模式测验(SDMT)评分差异均具有统计学意义(P均<0.05);RRMS组与NMOSD组各认知评分差异均无统计学意义(P均>0.05)。3组间丘脑及黑质QSV差异均有统计学意义(P均<0.05);相比NMOSD组及对照组,RRMS组丘脑QSV显著降低(P均<0.05)、黑质QSV显著升高(P均<0.05)。NMOSD组与对照组各灰质核团QSV差异均无统计学意义(P均>0.05)。RRMS组患者丘脑QSV与DST评分呈正相关(r=0.41,P=0.02)。结论 RRMS患者深部灰质存在明显铁紊乱,且与其认知障碍相关;NMOSD患者深部灰质铁含量尚无明显异常。
英文摘要:
      Objective To compare iron content in deep gray matter of relapsing-remitting multiple sclerosis (RRMS) and neuromyelitis optica spectrum disorder (NMOSD) patients and the correlations with motor and cognitive impairment with quantitative susceptibility mapping (QSM). Methods Totally 35 RRMS patients (RRMS group), 30 NMOSD patients (NMOSD group) and 30 healthy subjects(control group) were enrolled. Based on brain QSM, the quantitative susceptibility value (QSV) of each gray matter nucleus was obtained. Then QSV and functional scores were compared, and the relationships of the abnormal QSV of gray matter nucleus and motor and cognitive impairment were explored with correlation analysis. Results The scores of Montreal cognitive assessment (MoCA), digit span task (DST) and symbol digit modalities test (SDMT) were statistically different among 3 groups (all P<0.05). There was no significant difference of cognitive scores between RRMS group and NMOSD group (all P>0.05). Significantly differences of QSV of thalamus and substantia nigra were found among 3 groups (both P<0.05). QSV of thalamus in RRMS group significantly decreased, while of substantia nigra significantly increased compared that in NMOSD group and control group (all P<0.05). There was no significantly difference of QSV in gray matter nuclei between NMOSD group and control group (all P>0.05). QSV of thalamus in RRMS patients was positively correlated with DST score (r=0.41, P=0.02). Conclusion Obvious iron disorder existed in the deep gray matter of RRMS patients and related to cognitive impairment, while the iron content of deep gray matter in NMOSD patients were not significantly abnormal.
