      目的 观察剪切波弹性成像(SWE)于中期妊娠预测双胎妊娠自发性早产(SPTB)的价值。方法 前瞻性纳入86名产前超声诊断为双胎妊娠的孕妇,于中期妊娠分别采用常规超声及SWE测量宫颈长度(CL)及剪切波速度(SWV),观察其预测SPTB的效能。结果 最终51名孕妇发生SPTB (SPTB组)、35名未发生SPTB (非SPTB组);组间孕妇年龄、流产次数、采用促排卵受孕与否、CL及宫颈SWV差异均有统计学意义(P均<0.05)。流产史、促排卵受孕、CL及宫颈SWV均为SPTB的影响因素(P均<0.05);流产史和经促排卵受孕为SPTB的危险因素(OR均>1)。以26.30 mm为CL的最佳截断值,其于中期妊娠预测双胎妊娠SPTB的特异度为88.60%,敏感度为64.70%,曲线下面积(AUC)为0.85;以2.00 m/s为宫颈SWV的最佳截断值,其特异度为85.70%,敏感度为84.30%,AUC为0.90;二者AUC差异有统计学意义(Z=2.23,P=0.02)。结论 中期妊娠采用SWE测量宫颈SWV可有效预测双胎妊娠孕妇发生SPTB。

      Objective To observe the value of shear wave elastography (SWE) for predicting spontaneous preterm birth (SPTB) of twin pregnancy in the second trimester of pregnancy. Methods A total of 86 women with twin pregnancy detected with prenatal ultrasound were prospectively enrolled. The cervical length (CL) and shear wave velocity (SWV) were measured with conventional ultrasound and SWE in the second trimester of pregnancy, respectively, and the efficacy of them for predicting SPTB was observed. Results SPTB finally occurred in 51 pregnant women (SPTB group), not in the other 35 ones (non SPTB group). There were significant differences of age, abortions, ovulation induction, CL and cervical SWV between groups (all P<0.05). History of abortion, ovulation induction, CL and cervical SWV were interfering factors of SPTB. Histories of abortion and ovulation induction were the risk factors of SPTB (both OR>1). Taken 26.30 mm as the best cutoff value of CL, the specificity, sensitivity and area under the curve (AUC) for predicting SPTB of twin pregnancy in the second trimester of pregnancy was 88.60%, 64.70% and 0.85 (95%CI), respectively, while taken 2.00 m/s as the best cutoff value of cervical SWV, the specificity, sensitivity and AUC was 85.70%, 84.30% and 0.90 (95%CI), respectively.There was significant difference of AUC between SWV and CL (Z=2.23, P=0.02). Conclusion SWE could effectively predict SPTB of women with twin pregnancy in the second trimester of pregnancy.
