      目的 观察18F-FDG PET/CT用于评价脉络膜黑色素瘤(CM)的价值。方法 纳入22例临床疑诊为CM的患者,行18F-FDG PET/CT全身显像,进行影像学诊断并测量相关参数。以病理或最终临床诊断为标准,计算PET/CT诊断CM的效能。采用Spearman相关性分析评价最大标准摄取值(SUVmax)、平均标准摄取值(SUVmean)、肿瘤代谢体积(MTV)及病灶糖酵解总量(TLG)与病灶基底最大径、高度之间的相关性。结果 22例中,PET/CT诊断20例CM、2例眼球良性病变;其中14例经病理确诊CM,PET/CT诊断CM的敏感度、特异度及准确率分别为100%(14/14)、25.00%(2/8)及72.73%(16/22)。14例CM病灶CT均表现为边界清晰的高密度影,平均CT值(83.96±25.18) HU,病灶基底最大径为(1.46±0.50) cm,高度为0.66(0.50,1.28) cm;PET/CT显示病灶糖代谢均高于对侧眼球,SUVmax为5.10(3.44,7.88),SUVmean为3.62(2.37,5.23),MTV为0.68(0.49,1.29) cm3,TLG为2.69(1.22,6.08) g。病灶MTV、TLG与病灶基底最大径(r=0.607、0.664,P均<0.05)及高度(r=0.829、0.758,P均<0.05)均呈正相关;SUVmax、SUVmean与病灶基底最大径(r=0.493、0.343,P均>0.05)及高度(r=0.464、0.398,P均>0.05)均无显著相关性。结论 18F-FDG PET/CT用于评估CM敏感度高、准确率较好,但特异度有限。

      Objective To observe the value of 18F-FDG PET/CT for evaluation on choroidal melanoma (CM). Methods A total of 22 patients with clinical suspected CM were enrolled. 18F-FDG PET/CT whole-body imaging was performed, and relevant parameters were measured. Taken pathological or final clinical diagnosis as standard, the efficacy of PET/CT for diagnosing CM was calculated. Spearman correlation analysis was used to evaluate the correlation among the maximum standard uptake value (SUVmax), the mean standard uptake value (SUVmean), metabolic tumor volume (MTV), total lesion glycolysis (TLG) and the maximum diameter and height of lesion base. Results Among 22 patients, CM was diagnosed in 20 cases, while the other 2 were diagnosed benign ocular lesions with PET/CT, and 14 cases of CM were proved by pathology. The sensitivity, specificity and accuracy of 18F-FDG PET/CT for diagnosis of CM was 100% (14/14), 25.00% (2/8) and 72.73% (16/22), respectively. Among 14 CM patients, the lesions showed high density shadow on CT with clear boundary, and the average CT value was (83.96±25.18)HU, the maximum diameter and the height of lesion base was (1.46±0.50) cm and 0.66 (0.50, 1.28) cm, respectively. PET/CT showed that glucose metabolism of CM lesions were higher than that of the contralateral eyeballs, with SUVmax of 5.10 (3.44, 7.88), SUVmean of 3.62 (2.37, 5.23), MTV of 0.68 (0.49, 1.29)cm3 and TLG of 2.69 (1.22, 6.08)g. MTV and TLG of CM lesions were positively correlated with the maximum basal diameter (r=0.607, 0.664, both P<0.05) and the height (r=0.829, 0.758, both P<0.05) of the base of lesions, whereas SUVmax and SUVmean were not significantly correlated with the maximum basal diameter (r=0.493, 0.343, both P>0.05) and the height (r=0.464, 0.398, both P>0.05) of lesions. Conclusion 18F-FDG PET/CT had high sensitivity and good accuracy for diagnosing CM, but its specificity was rather limited.
