目的 采用静息态功能MRI(rs-fMRI)观察终末期肾病(ESRD)患者前扣带回皮质(ACC)功能连接特点及其与认知功能的相关性。方法 采集29例ESRD患者(ESRD组)及29名健康志愿者(对照组)的颅脑rs-fMRI, 并评价其认知功能。以双侧ACC为ROI, 分析与ACC功能连接存在组间差异的脑区; 以Pearson相关性分析观察ESRD组差异脑区功能连接强度与认知评分的相关性。结果 组间认知功能评价结果差异均具有统计学意义(P均<0.05)。相比对照组, ESRD组左侧枕叶外侧皮质、双侧额中回、双侧额极及双侧中央后回与左侧ACC功能连接增加(P均<0.005), 双侧丘脑、双侧内侧前额叶皮质(MPFC)及双侧后扣带回则减低(P均<0.005);双侧额中回、双侧额极及左侧枕叶外侧皮质与右侧ACC功能连接增加(P均<0.005), 双侧丘脑及双侧MPFC减低(P均<0.005)。右侧ACC和MPFC功能连接与数字符号替换试验(DSST)评价结果呈显著正相关(r=0.428, P<0.05);其他脑区功能连接强度与认知评价均无明显相关(P均>0.05)。结论 ESRD患者ACC与默认模式网络(DMN)、执行控制网络、感觉网络及视觉网络等构成脑区存在功能连接异常; ACC和MPFC的功能连接与ESRD患者认知执行控制功能密切相关。 |

Objective To observe the functional connectivity of anterior cingulate cortex (ACC) and correlations with cognition function in patients with end-stage renal disease (ESRD) with resting-state functional MRI (rs-fMRI).Methods Totally 29 ESRD patients (ESRD group) and 29 healthy volunteers (control group) underwent rs-fMRI scanning and cognitive function tests. Bilateral ACC was taken as ROI to analyze brain areas with different functional connections between groups. Pearson analysis was used to evaluate the correlations of brain areas with different functional connectivity and cognitive scores in ESRD group.Results Significant differences of cognitive function were found between groups (all P < 0.05). Compared with those in control group, the areas of increased functional connectivity to left ACC in ESRD group included left lateral occipital cortex, bilateral middle frontal gyrus, bilateral frontal pole and bilateral postcentral gyrus (all P < 0.005), and the decreased areas included bilateral thalamus, bilateral medial prefrontal cortex (MPFC) and bilateral posterior cingulate cortex (all P < 0.005), while the areas with increased functional connectivity to right ACC included bilateral middle frontal gyrus, bilateral frontal pole and left lateral occipital cortex (all P < 0.005), and the decreased areas included bilateral thalamus and bilateral MPFC (all P < 0.005). Significant positive correlation was found between ACC and MPFC functional connection and digit symbol substitution test (DSST) evaluation (r=0.428, P < 0.05), but not between functional connection strength of other brain areas and cognitive evaluation (all P > 0.05).Conclusion The functional connectivity of ACC with default mode network (DMN), executive control network, sensory network and visual network were abnormal in patients with ESRD, and the abnormal functional connectivity between right ACC and MPFC was closely associated with cognitive executive control function. |
