目的 观察心血管MR特征追踪(CMR-FT)技术评估糖尿病(DM)前期、DM患者右心室亚临床功能改变的价值。方法 纳入40例DM前期(DM前期组)、27例DM患者(DM组)及30名健康人(对照组), 比较组间空腹血糖(FBG)及糖化血红蛋白(HbA1c)检测结果差异, 测量并比较3组间基于MRI的右心室容积、功能及应变参数差异。采用组内相关系数(ICC)评价观察者内及观察者间测量参数的一致性; 以Spearman相关系数评价HbA1c与右心室参数的相关性。结果 DM前期组和DM组FBG、HbA1c均显著高于对照组(P均<0.05), DM组FBG、HbA1c均显著高于DM前期组(P均<0.05)。观察者内及观察者间测量右心室各参数结果的一致性均良好(ICC均>0.75, P均<0.05)。3组间各容积与功能参数差异均无统计学意义(P均>0.05)。DM组右心室整体径向应变(GRS)、整体圆周应变(GCS)及收缩期峰值径向应变率(PSSR R)均明显高于对照组(P均<0.05), 整体纵向应变(GLS)明显低于对照组(P<0.05);DM前期组PSSR R明显高于对照组(P<0.05);DM组GCS明显高于DM前期组(P<0.05), GLS明显低于DM前期组(P<0.05)。HbA1c与GCS、GLS、PSSR R及收缩期峰值圆周应变率(PSSR C)均呈弱相关(r=-0.207、0.241、0.254、-0.207, P均<0.05)。结论 CMR-FT技术可用于评估DM前期和DM患者亚临床右心室功能改变。 |

Objective To observe the value of cardiovascular MR feature tracking (CMR-FT) for evaluation on the changes of right ventricular function in patients with prediabetes (pre-DM) and diabetes mellitus (DM).Methods A total of 40 pre-DM patients (pre-DM group), 27 DM patients (DM group) and 30 healthy subjects (control group) were enrolled. The result of fasting blood glucose (FBG) and glycosylated hemoglobin (HbA1c) were compared among groups. The volume, function and strain parameters of the right ventricle based on MRI were measured and compared among 3 groups. The inter-class correlation coefficient (ICC) was used to assess both inter- and intra-observer variabilities. Spearman analysis was performed to assess the correlations of HbA1c level and right ventricular parameters.Results FBG and HbA1c levels were significantly higher in pre-DM group and DM group than those in control group (all P < 0.05). FBG and HbA1c in DM group were significantly higher than those in pre-DM group (both P < 0.05). The consistencies of intra-observer and inter-observer measurements of right ventricular parameters were all good (all ICC > 0.75, all P < 0.05). There was no significant difference of volume nor functional parameters of the right ventricle among 3 groups (all P > 0.05). The global radial strain (GRS), global circumferential strain (GCS) and peak systolic radial strain rate (PSSR R) of the right ventricle in DM group were significantly higher than those in control group (all P < 0.05), while the global longitudinal strain (GLS) in DM group was significantly lower than that in control group (P < 0.05). PSSR R in pre-DM group was significantly higher than that in control group (P < 0.05). GCS in DM group was significantly higher than that in pre-DM group (P < 0.05), and GLS was significantly lower than that in pre-DM group (P < 0.05). HbA1c was weakly correlated with GCS, GLS, PSSR R and peak systolic circumferential strain rate (PSSR C) (r=-0.207, 0.241, 0.254, -0.207, all P < 0.05).Conclusion CMR-FT could be used to evaluate the subclinical changes of right ventricular function of pre-DM and DM patients. |
