目的 观察心脏MRI成像(CMRI)评估以阿霉素诱导卵巢切除大鼠模型心肌毒性的价值。方法 将48只SD大鼠随机分为阿霉素高剂量治疗组(高剂量组)、阿霉素低剂量治疗组(低剂量组)、卵巢切除组及假手术组, 每组12只。切除前3组大鼠双侧卵巢。对高剂量组和低剂量组分别以2.5 mg/kg和1.5 mg/kg体质量静脉注射阿霉素, 每周1次, 共6周, 建立心肌毒性模型; 对其余2组注射生理盐水。分别于造模前(第0周)、阿霉素注射后每周(第1~6周)及注药结束后间隔2周行CMRI至第12周, 比较组间左心室射血分数(LVEF)、左心室整体径向应变(LVGRS)、左心室整体周向应变(LVGCS)、左心室整体纵向应变(LVGLS)及左心室标准化心肌T2信号强度比值(T2 SIr)的差异。结果 相比卵巢切除组和假手术组, 高剂量组及低剂量组LVEF分别于第8周及第12周开始下降, LVGRS、LVGCS及LVGLS绝对值分别于第6周及第8周开始降低, 左心室T2 SIr分别于第6周及第8周达峰(P均<0.05)。相比高剂量组, 低剂量组LVEF于第8周, LVGRS和LVGLS于第6周始上升, LVGCS仅于第8周升高, 第6、8、10周左心室T2 SIr差异均有统计学意义(P均<0.05);卵巢切除组与假手术组间各时间点各参数差异均无统计学意义(P均>0.05)。结论 CMRI心肌应变和T2 mapping有助于早期评估卵巢切除后大鼠模型阿霉素诱导心肌毒性。 |

Objective To observe the value of cardiac MRI (CMRI) in evaluation on doxorubicin-induced cardiotoxicity in ovariectomized rat models.Methods Forty-eight female SD rats were randomly divided into high-dose doxorubicin treatment group (high-dose group), low-dose doxorubicin treatment group (low-dose group), ovariectomized group and sham group (each n=12). Bilateral ovariectomy was performed in rats in high- and low-dose groups and ovariectomized group. Then 2.5 mg/kg and 1.5 mg/kg body weight doxorubicin were injected in rats in high- and low-dose groups once per week for 6 weeks, respectively, to establish myocardial toxicity models, while normal saline was injected in rats in the rest 2 groups CMRI was performed before modeling (week 0), every week after injection (1 to 6 weeks) and then every 2 weeks after injection (8 to 12 weeks). Left ventricular ejection fraction (LVEF), left ventricular global radial strain (LVGRS), left ventricular global circumstantial strain (LVGCS), left ventricular global longitudinal strain (LVGLS) and left ventricle normalized the myocardial T2 signal intensity ratio (T2 SIr) were compared among groups.Results Compared with ovariectomized group and sham group, LVEF of high- and low-dose groups began to decrease 8 and 12 weeks later, respectively, and LVGRS, LVGCS and LVGLS decreased 6 and 8 weeks later, respectively (all P < 0.05), while left ventricular T2 SIr reached the peak 6 and 8 weeks later, respectively (all P < 0.05). Compared with high-dose group, LVEF, LVGRS and LVGLS began to increase 6 weeks later in low-dose group, while LVGCS only increased at 8 weeks later. There were statistical differences of left ventricular T2 SIr 6, 8 and 10 weeks later (all P < 0.05). No significant difference of parameters at each time point was found between ovariectomy group and sham group (all P > 0.05).Conclusion CMRI myocardial strain and T2 mapping were helpful for early assessment of doxorubicin-induced cardiotoxicity in ovariectomized rat models. |
