      目的 采用T2* mapping技术评估男性业余马拉松运动员完成半程马拉松后半月板改变。方法 招募16名男性业余马拉松运动员,分别于半程马拉松运动前、运动后3 h、3天及1周行膝关节MR检查,共获得30侧膝关节(16侧右膝,14侧左膝)MRI;测量内、外侧半月板前角、后角及体部T2*值,以组内相关系数(ICC)评价观察者两次测量结果的一致性;比较运动前后各时间点膝关节半月板T2*值变化。结果 T2*测量值观察者间一致性良好(ICC=0.817,P<0.05)。运动前膝关节内侧半月板T2*值显著高于外侧半月板(P<0.05)。运动后3 h、3天膝关节半月各个区域T2*值均显著高于运动前(P均<0.05);运动后3天内侧半月板体部T2*值显著高于运动后3 h(P<0.05);运动后1周半月板各区域T2*值均显著低于运动后3天(P均<0.05),而与运动前差异均无统计学意义(P均>0.05);运动后1周外侧半月板前角及后角T2*值均显著低于运动后3 h(P均<0.05)。结论 采用T2* mapping技术可较为敏感地评估半月板变化;半程马拉松运动后,男性业余马拉松运动员膝关节半月板T2*值变化为可逆性改变,可能与其成分改变相关。
英文摘要:
      Objective To observe meniscus changes of male amateur marathon after half marathon with T2* mapping. Methods A total of 16 male amateur marathon runners were recruited. MR examinations of knee joints were performed before running and 3 h, 3 days and 1 week after half marathon. MR images of 30 knee (16 right knees and 14 left knees) were obtained. T2* values of the anterior horn, posterior horn and body of the medial and lateral meniscus were measured, respectively. The consistency of two times of measurements was evaluated using intra-class correlation coefficient (ICC). The changes of T2* value at each time point in meniscus of knee joint before and after half marathon were compared. Results Good intra-observer agreement of T2* was found between two times of measurements (ICC=0.817, P<0.05). T2* value of medial meniscus was significantly higher than that of lateral meniscus before running (P<0.05). Three hours and 3 days after half marathon, T2* values of each area of knee joint were significantly higher than those before running (all P<0.05). T2* value of medial meniscus body 3 days after half marathon was significantly higher than that of 3 h after running (P<0.05), each meniscus area 1 week after half marathon were significantly lower than those 3 days after running (all P<0.05) but not significantly different compared with those before running (all P>0.05). T2* values of anterior and posterior horn of medial and lateral meniscus 1 week after half marathon were significantly lower than those of 3 h after running (all P<0.05). Conclusion T2* mapping technique could be used to sensitively assess changes of meniscus. T2* changes of meniscus in male amateur marathon runners after half marathon were reversible, which might be related to the components changes of meniscus.
