目的 比较18F-FDG PET/CT与PET/MRI诊断胰腺占位性病变的价值。方法 回顾性分析34例接受体部18F-FDG PET/CT检查及上腹部MR扫描的胰腺占位患者,对比观察18F-FDG PET/CT与PET/MRI判断良恶性病灶、图像融合匹配度、图像质量及图像附加信息,评估二者诊断胰腺占位性病变的价值。结果 34例共检出37个胰腺病灶。PET/CT诊断7个良性病灶、30个恶性病灶,其诊断胰腺癌的敏感度、特异度和准确率分别为95.83%、46.15%和78.38%;PET/MRI诊断11个良性病灶、26个恶性病灶,其诊断胰腺癌的敏感度、特异度和准确率分别为100%、84.61%和94.59%,诊断准确率显著高于PET/CT(χ2=4.16,P=0.04)。PET/CT与PET/MR图像融合匹配度差异无统计学意义(P>0.05)。CT、MR、PET/CT及PET/MR图像质量差异无统计学意义(P>0.05)。27.03%(10/37)的图像附加信息评分为-1分,8.11%(3/37)为0分,64.86%(24/37)为1分。结论 PET/MRI诊断胰腺癌的敏感度、特异度、图像融合匹配度及图像质量均与PET/CT相近,而其诊断准确率高于PET/CT,有助于诊断及鉴别胰腺占位性病变。 |

Objective To compare the value of 18F-FDG PET/CT and PET/MRI for diagnosing space-occupying pancreatic lesions. Methods Data of 34 patients with space-occupying pancreatic lesions who underwent whole-body 18F-FDG PET/CT and upper abdominal MR examination were retrospectively analyzed. The nature of lesions, image fusion matching degree, image quality and additional information of 18F-FDG PET/CT and PET/MRI were observed and compared, and the values of diagnosing space-occupying pancreatic lesions were assessed. Results Totally 37 lesions were detected in 34 patients. PET/CT diagnosed 7 benign lesions and 30 malignant ones, the sensitivity, specificity and accuracy of PE/CT for diagnosing pancreatic cancer was 95.83%, 46.15% and 78.38%, respectively. PET/MRI diagnosed 11 benign lesions and 26 malignant ones, the sensitivity, specificity and accuracy was 100%, 84.61% and 94.59%, respectively. The diagnostic accuracy of PET/MRI was significantly higher than that of PET/CT (χ2=4.16, P=0.04). There was no significant difference of image fusion matching degrees between PET/CT and PET/MRI (P>0.05), nor of image quality among CT, MRI, PET/CT and PET/MRI (P>0.05). The additional image information score of 27.03% (10/37)lesions was -1, of 8.11% (3/37) was 0, while of 64.86% (24/37) was 1. Conclusion The sensitivity and specificity for diagnosing pancreatic cancer, image fusion matching degree and image quality of PET/MRI were similar to those of PET/CT, while the diagnostic accuracy of PET/MRI was higher than that of PET/CT, hence being helpful for diagnosing and differential space-occupying pancreatic lesions. |
