      目的 对比观察PET/MRI与PET/CT检出肝转移癌的效能。方法 纳入78例共409个肝转移癌病灶,比较PET/CT及PET/MRI检出肝转移癌的阳性率。对A组50例283个病灶先行PET/CT后行PET/MR检查,B组28例126个病灶先行PET/MR后行PET/CT。采用Spearman秩相关分析和Wilcoxon秩和检验评价组内不同检查方法所获病灶最大标准摄取值(SUVmax)的相关性及其差异。结果 PET/MRI对肝转移癌的检出率(381/409,93.15%)明显高于PET/CT(285/409,69.68%,P<0.05)。A组PET/MRI的阳性率(266/283,93.99%)明显高于PET/CT(197/283,69.61%,P<0.05);PET/MRI与PET/CT的SUVmax高度相关(r=0.84,P<0.05),而PET/MRI的SUVmax显著高于PET/CT的SUVmaxP<0.05)。B组PET/MRI阳性率(115/126,91.27%)明显高于PET/CT(88/126,69.84%,P<0.05);PET/MRI与PET/CT的SUVmax中度相关(r=0.74,P<0.05),且差异无统计学意义(P>0.05)。结论 PET/MRI检出肝转移癌效果优于PET/CT;二者所获SUVmax相关性较好。

      Objective To observe the efficacy of PET/MRI and PET/CT for detecting liver metastases. Methods A total of 409 liver metastases were observed in 78 patients, the positive rates of PET/CT and PET/MRI for detecting liver metastases were compared. Fifty patients (283 lesions) in group A underwent first PET/CT and then PET/MR, while 28 patients (126 lesions) in group B underwent first PET/MR and then PET/CT. Spearman rank correlation analysis and Wilcoxon signed rank test were performed to compare the correlations and differences of the maximum standard uptake value (SUVmax) of liver metastases obtained with two methods in each group. Results The positive rate of PET/MRI for detecting liver metastases (381/409, 93.15%) was significantly higher than that of PET/CT (285/409, 69.68%, P<0.05). In group A, the positive rate of PET/MRI (266/283, 93.99%) for detecting liver metastases was significantly higher than that of PET/CT (197/283, 69.61%, P<0.05), and SUVmax obtained with PET/MRI was highly correlated with that obtained with PET/CT (r=0.84, P<0.05), while SUVmax obtained with PET/MRI was significantly higher than that obtained with PET/CT (P<0.05). In group B, the positive rate of PET/MRI (115/126, 91.27%) was significantly higher than that of PET/CT (88/126,69.84%, P<0.05), and SUVmax obtained with PET/MRI was moderately correlated with that obtained with PET/CT (r=0.74, P<0.05), but no significant difference of SUVmax was found between PET/CT and PET/MRI (P>0.05). Conclusion PET/MRI was better than PET/CT for detecting liver metastases, and good correlation existed between SUVmax obtained with PET/MRI and PET/CT.
