目的 观察动态向量血流成像(V Flow)技术评价肌少症患者颈动脉管腔内流场状态及血流动力学变化的价值。方法 纳入120名 ≥ 65岁老年人,分为肌少症组(n=58)及对照组(n=62)。采用二维超声测量双侧颈动脉内-中膜厚度(IMT),计数颈动脉斑块数目,评估颈动脉狭窄程度;以V Flow技术获取颈动脉ROI,测量双侧颈动脉血流速度及管壁剪切应力(WSS)。比较组间一般资料、临床资料及超声参数差异,以Pearson相关分析评价各指标的相关性。结果 肌少症组体质量指数(BMI)及骨骼肌质量指数(RASMI)均明显低于对照组(P均<0.05),甘油三酯(TG)及总胆固醇(TC)均明显高于对照组(P均<0.05)。肌少症组双侧颈动脉IMT高于对照组(P<0.05),WSS低于对照组(P<0.05),颈动脉斑块数目、颈动脉狭窄程度 ≥ 50%例数均多于对照组(P均<0.05)。颈动脉WSS与年龄、收缩压及IMT均呈负相关(r=-0.469、-0.593、-0.762,P均<0.05),与ROI流速呈正相关(r=0.291,P<0.05)。结论 V Flow技术可为评估肌少症患者颈动脉改变提供依据,应用价值良好。 |

Objective To observe the value of vector blood flow imaging (V Flow) for evaluation on flow field state and hemodynamic changes of carotid artery in patients with sarcopenia. Methods A total of 120 elderly people (≥ 65 years old) were enrolled and divided into sarcopenia group (n=58) and control group (n=62). Bilateral carotid artery intima media thicknesses (IMT) were measured, the numbers of carotid plaques were counted, and the degrees of carotid artery stenosis were assessed with two-dimensional ultrasound. Then ROI of carotid arteries were obtained with V Flow technique, bilateral blood flow velocities and wall shear stresses (WSS) were measured. The general data, clinical data and ultrasonic parameters were compared between the groups. Pearson correlation analysis was used to analyze the correlation of the parameters. Results The body mass index (BMI) and relative appendicular skeletal muscle mass index (RASMI) of the sarcopenia group were both significantly lower than those of the control group (both P<0.05), while triglyceride (TG) and total cholesterol (TC) in sarcopenia group were both significantly higher than those in control group (both P<0.05). In sarcopenia group, IMT of bilateral carotid arteries were higher than that of control group (P<0.05), while WSS were lower than that of the control group (P<0.05), and the numbers of carotid plaques and the degree of carotid artery stenosis ≥ 50% were more than those in control group (both P<0.05). WSS of carotid arteries were negatively correlated with age, systolic blood pressure and IMT (r=-0.469, -0.593, -0.762, all P<0.05) and positively correlated with ROI flow rate (r=0.291, P<0.05). Conclusion V Flow technology could provide evidences for evaluating changes of carotid artery in sarcopenia patients, therefore having good application value. |
