      目的 评价超微血管成像(SMI)检测血流分布及内部血管特征诊断甲状腺影像报告和数据系统(TI-RADS)4级甲状腺结节的价值。方法 纳入115例TI-RADS 4级甲状腺结节患者共123个结节,采用SMI及CDFI模式观察结节血流分布和内部血管特征,比较二者鉴别诊断TI-RADS 4级甲状腺良恶性结节的敏感度、特异度及准确率。结果 69个良性结节以Ⅱ型血流为主,54个恶性结节以Ⅲ型血流为主。SMI对良性结节Ⅱ型及恶性结节Ⅲ型血流显示率均明显高于CDFI(P均<0.01)。SMI及CDFI诊断甲状腺良、恶性结节的敏感度分别为92.59%和72.22%,特异度分别为89.85%和81.16%,准确率分别为91.06%和77.24%。SMI对恶性结节内走行扭曲血管和穿支血管的检出率明显高于CDFI(P均<0.01)。结论 甲状腺TI-RADS 4级良恶性结节血流分布模式存在差异;SMI可为鉴别甲状腺良恶性结节提供更多诊断依据。

      Objective To evaluate the value of superb micro-vascular imaging (SMI) for detecting blood flow distribution and internal vascular features of thyroid imaging report and data system (TI-RADS) grade 4 thyroid nodules. Methods A total of 115 patients with 123 nodules TI-RADS grade 4 thyroid nodules were enrolled. Blood flow distribution and internal vascular characteristics of thyroid nodules were observed with SMI and CDFI, and the sensitivity, specificity and accuracy for differentiating TI-RADS grade 4 benign and malignant thyroid nodules of CDFI and SMI were compared. Results Among 69 benign nodules, Ⅱ type blood flow was the most common, while in 54 malignant nodules, Ⅲ type blood flow was the common. SMI detecting rate of benign nodules with typeⅡ blood flow and malignant nodules with type Ⅲ blood flow were significantly higher than that of CDFI (both P<0.01). The sensitivity of SMI and CDFI for diagnosing benign and malignant TI-RADS grade 4 thyroid nodules were 92.59% and 72.22%, the specificity were 89.85% and 81.16%, and the accuracy were 91.06% and 72.24%, respectively. SMI detection rate of deformed vessels and perforator vessels within malignant nodules were significantly higher than that of CDFI (both P<0.01). Conclusion Benign and malignant thyroid TI-RADS grade 4 nodules had different blood flow patterns. SMI could provide diagnostic basis for differentiating benign and malignant TI-RADS grade 4 thyroid nodules.
