      目的 观察基于有效直径(De)和水当量直径(Dw)获得儿童颈部CT扫描体型特异性剂量评估(SSDE)值的价值。方法 回顾性分析232例接受颈部CT检查患儿,依据年龄分为A组(0~5岁,n=77)、B组(6~9岁,n=79)及C组(10~16岁,n=76)。记录容积CT剂量指数(CTDIVOL),计算并比较3组De、Dw,转换因子(fDe及fDw),SSDEDe及SSDEDw结果 3组fDe均大于fDwt=22.37、34.52、12.38,P均<0.05),SSDEDe均大于SSDEDwt=18.21、20.00、25.11,P均<0.05)。A组SSDEDe和SSDEDw较CTDIVOL分别增高33.88%和26.86%,SSDEDe较SSDEDw高5.54%;B组SSDEDe和SSDEDw较CTDIVOL分别高21.05%和10.18%,SSDEDe较SSDEDw高9.87%;C组SSDEDe和SSDEDw较CTDIVOL分别低3.98%和8.55%,SSDEDe较SSDEDw高4.98%。结论 相比基于De获得的SSDE,基于Dw获得的SSDE能更精确地评估儿童颈部CT检查辐射剂量。

      Objective To explore the application value of size-specific dose estimate (SSDE) values based on effective diameter (De) and water equivalent diameter (Dw) in evaluation on radiation dose in children neck CT examination. Methods Data of 232 children who underwent neck CT were retrospectively analyzed. The children were divided into group A (0-5 years old, n=77), B (6-9 years old, n=79) and C (10-16 years old, n=76) according to ages. The volume CT dose index (CTDIVOL) were recorded, and the De, Dw, conversion factors (fDe and fDw), SSDEDe and SSDEDw were calculated and compared among 3 groups. Results In all 3 groups, fDe was larger than fDw (t=22.37, 34.52, 12.38, all P<0.05), and SSDEDe was higher than SSDEDw (t=18.21, 20.00, 25.11, all P<0.05). In group A, SSDEDe and SSDEDw were 33.88% and 26.86% higher than CTDIVOL, respectively, and SSDEDe was 5.54% higher than SSDEDw. In group B, SSDEDe and SSDEDw were 21.05% and 10.18% higher than CTDIVOL, respectively, and SSDEDe was 9.87% higher than SSDEDw. In group C, SSDEDe and SSDEDw were 3.98% and 8.55% lower than CTDIVOL, respectively, and SSDEDE was 4.98% higher than SSDEDw. Conclusion Compared with SSDE based on De, SSDE based on Dw could more accurately assess radiation dose of children neck CT examination.
