目的 探讨声辐射力脉冲(ARFI)弹性成像定量技术无创评估移植肾术后不同状态的价值。方法 对95例肾移植患者行ARFI弹性成像检查,其中30例移植肾功能稳定(肾功能稳定组),65例移植肾功能受损且接受穿刺活检患者(肾功能障碍组,包括急性排斥亚组37例、药物毒性亚组15例及病毒感染亚组13例);比较组间超声指标差异。结果 肾功能稳定组与肾功能障碍组间剪切波速度(SWV)差异无统计学意义(P>0.05),肾功能稳定组与肾功能障碍3个亚组段间动脉、叶间动脉阻力指数(RI)差异均无统计学意义(P均>0.05)。肾功能障碍组内急性排斥亚组和病毒感染亚组SWV均高于药物毒性亚组及肾功能稳定组(P均<0.05)。结论 ARFI弹性成像定量技术可用于鉴别移植肾术后状态,为临床提供参考。 |

Objective To explore the value of acoustic radiation force impulse (ARFI) elastography quantitative technique for evaluating post-operative status of transplanted kidney. Methods A total of 95 patients after renal transplantation underwent ARFI elastography quantitative examination, 30 with stable renal function (stable renal function group) and 65 with impaired renal function and underwent biopsy (renal dysfunction group, including 37 cases in acute rejection subgroup, 15 cases in drug toxicity subgroup and 13 cases in virus infection subgroup). The ultrasonic indexes were compared between groups and among group-subgroups. Results There was no significant difference in SWV between stable renal function group and renal dysfunction group (P>0.05), there was no significant difference in resistance index (RI) of inter-segment artery and inter-lobar artery between stable renal function group and three subgroups of renal dysfunction (all P>0.05). SWV in acute rejection subgroup and virus infection subgroup were higher than that in stable renal function subgroup, drug toxicity subgroup and stable renal function group (all P<0.05). Conclusion ARFI elastography quantitative technology could be used to identify the status of transplanted kidney and provide references for clinical work. |
