目的 评价经阴道二维超声(2D-TVUS)与经阴道三维超声(3D-TVUS)诊断宫腔粘连(IUA)的临床价值。方法 搜索中国知网、万方医学网、维普数据库、OVID、Medline、SinoMed、PubMed、Cochrane library、Embase等数据库,搜集2D-TVS与3D-TVS诊断宫腔粘连疾病文献,检索时间为建库至2020年2月27日。由2名研究人员独立筛选文献、提取信息并以RevMan 5.3评价纳入文献偏倚风险,以Stata 16.0和Meta Disc1.4软件进行Meta分析。结果 共纳入19项研究,2D-TVUS敏感度(SEN)、特异度(SPE)、阳性似然比(PLR)、阴性似然比(NLR)及诊断比值比(DOR)分别为0.74[95% CI(0.70,0.78)]、0.69[95% CI(0.51,0.82)]、2.4[95% CI(1.4,4.0)]、0.37[95% CI(0.28,0.50)]及6[95% CI(3,14)],3D-TVUS分别为0.88[95% CI(0.85,0.91)]、0.88[95% CI(0.75,0.94)]、7.1[95% CI(3.4,14.7)]、0.13[95% CI(0.10,0.17)]及0.13[95% CI(0.10,0.17)]。2D-TVUS、3D-TVUS和二者联合的受试者工作特征(ROC)曲线下面积(AUC)分别为0.77、0.92和0.93。结论 3D-TVUS诊断宫腔粘连的效能显著高于2D-TVUS,以及二者联合诊断效能最高。 |

Objective To evalude the clinical value of transvaginal two-dimensional ultrasound (2D-TVUS) and transvaginal three-dimensional ultrasound (3D-TVUS) in diagnosis of intrauterine adhesions (IUA).Methods CNKI, Wanfang Med Online, VIP databases, OVID, Medline, SinoMed, PubMed, Cochrane library and Embase were searched. Diagnostic studies of intrauterine adhesions with 2D-TVUS and 3D-TVUS were collected. The retrieval time was from the establishment of database to February 27, 2020. Two researchers independently screened the literature, extracted information and evaluated the risk of bias of included articles using Revman 5.3. Stata 16.0. Meta Disc 1.4 software was used for meta-analysis.Results A total of 19 studies were included. The sensitivity (SEN), specificity (SPE), positive likelihood ratio (PLR), negative likelihood ratio (NLR) and diagnostic odds ratio (DOR) of 2D-TVUS was 0.74[95%CI (0.70,0.78)], 0.69[95%CI (0.51, 0.82)], 2.4[95%CI (1.4, 4.0)], 0.37[95%CI (0.28, 0.50)] and 6[95%CI (3, 14)], of 3D-TVUS was 0.88[95%CI (0.85, 0.91)], 0.88[95%CI (0.75, 0.94)], 7.1[95%CI (3.4, 14.7)], 0.13[95%CI (0.10, 0.17)] and 0.13[95%CI (0.10, 0.17)],respectively. The area under the receiver operating characteristic (ROC) curve (AUC) of 2D-TVUS, 3D-TVUS and the combination of 2D- and 3D-TVUS was 0.77, 0.92 and 0.93, respectively.Conclusion The diagnostic value of 3D-TVUS for intrauterine adhesions was significantly higher than that of 2D-TVUS, and the combination of 2D- and 3D-TVUS had the highest diagnostic efficiency. |
