      目的 观察胎儿不同类型左无名静脉异常的超声心动图特征及其临床意义。方法 回顾性分析67胎左无名静脉异常胎儿的临床资料、超声心动图特征及预后。结果 ①67胎中,47胎左无名静脉走行异常,12胎主动脉弓上方左无名静脉缺如,8胎左无名静脉内径异常或异常血流。②47胎左无名静脉走行异常中,左无名静脉主动脉弓下走行40胎、食管后(或气管后)走行5胎,双左无名静脉1胎,左无名静脉胸腺内走行1胎。③40胎主动脉弓下左无名静脉中,孤立型18胎,22胎合并心脏畸形,包括右位主动脉弓伴左锁骨下动脉迷走、右位主动脉弓伴镜像分支、室间隔缺损、双主动脉弓及法洛四联症等。结论 产前超声心动图可清晰显示胎儿左无名静脉位置、走行及血流信号等;了解胎儿左无名静脉异常类型及合并畸形特点有助于提高产前正确诊断率,并为临床决策提供参考。
英文摘要:
      Objective To observe prenatal echocardiographic characteristics and clinical significances of different type fetal left innominate vein abnormalities. Methods The clinical, echocardiographic and prognosis data of 67 fetuses with abnormal left innominate veins diagnosed with fetal echocardiography were retrospectively analyzed. Results ①Among 67 fetuses, abnormal route of left innominate vein was found in 47 fetuses, while the other 12 were detected with absent left innominate vein and 8 with abnormal diameter of left innominate vein or abnormal blood flow in left innominate vein. ②Among 47 fetuses with abnormal route of left innominate vein, the left innominate vein trended under aortic arch in 40 fetuses, while retroesophageal or retrotracheal left innominate vein were observed in 5 fetuses, double left innominate vein in 1 fetus and left innominate vein penetrating thymus in 1 fetus. ③Among 40 fetuses of left innominate vein under the aortic arch, 18 were found without cardiac malformations, whereas 22 were detected with complicated cardiac malformations including right aortic arch with left subclavian artery vagal, right aortic arch with mirror branch, ventricular septal defect, double aortic arch, tetralogy of Fallot, etc. Conclusion Prenatal echocardiography could clearly show the position, course and blood flow signals of fetal left innominate vein. Understanding the types of fetal left innominate vein abnormalities and the characteristics of associated malformations were helpful to improving prenatal diagnosis and providing references for clinical decision-making.
