目的 观察Revolution CT单能量成像评估腰椎旁肌退行性病变的价值。方法 回顾性分析124例接受Revolution CT腰椎平扫患者,分为青年组(20~39岁,40例)、中年组(40~59岁,42例)及老年组(60~89岁,42例),比较不同年龄、性别及椎间隙水平竖脊肌(ES)、多裂肌(MF)及腰大肌(PS) CT值差异,分析年龄、性别及部位(以椎间隙水平为代表)对腰椎旁肌CT值的影响。结果 ES、MF及PS的CT值均随年龄增长而下降(P均<0.01)。青、中年组男性各椎旁肌CT值均高于女性(P均<0.05),老年组男性ES的CT值高于女性(P=0.01)。各组内不同部位ES及中、老年组内不同部位MF的CT值差异均有统计学意义(P均<0.05),不同部位PS的CT值差异均无统计学意义(P均>0.05)。年龄、性别及部位均与ES、MF及PS的CT值有关(P均<0.01),以年龄影响最著。结论 Revolution CT单能量成像可用于评估腰椎旁肌退行性病变;年龄、性别及部位对腰椎旁肌退行性病变存在一定影响。 |

Objective To observe the value of Revolution CT single-energy imaging for assessment of lumbar paravertebral muscles degenerative change. Methods Data of 124 patients who underwent lumbar spine examinations were retrospectively analyzed. The patients were divided into young age group (20-39 years old, n=40), middle age group (40-59 years old, n=42) and elderly group (60-89 years old, n=42). CT values of erector spinae (ES), multifidus muscle (MF) and psoas major (PS) at different ages, genders and intervertebral levels (on behalf of different positions of spine)were compared, and the impact of age, gender and intervertebral level on CT values of lumbar paraspinal muscles were analyzed. Results CT values of ES, MF and PS decreased with aging (all P<0.01). Significant sex-dependent differences of ES, MF and PS were found between the young and middle age groups, but all higher in men than in women (all P<0.05). In elderly group, CT value of ES was higher in men than in women (P=0.01). There were statistical differences of CT values of ES at different intervertebral levels in different groups (all P<0.05). CT values of MF at different intervertebral levels in middle and elderly groups were significantly different (all P<0.05), but no significant difference of CT values of PS at different intervertebral levels was detected in each age group (all P>0.05). Aging, gender and intervertebral space level were all correlated with CT values of ES, MF and PS (all P<0.01), among which aging had the greatest impact on CT values of paraspinal muscles. Conclusion Revolution CT single-energy imaging could be used to evaluate the degenerative change of lumbar paravertebral muscles. Aging, gender and intervertebral level had certain impact on lumbar paraspinal muscle degenerative change. |
