目的 观察健康成人上肢肱动脉(BA)至指端微动脉血流动力学变化特点及性别差异。方法 纳入205名健康志愿者,男性组88名,女性组117名。以常规彩色多普勒超声结合平面波超声敏感血流显像(Angio PLUS)观察右上肢BA、尺动脉(UA)、桡动脉(RA),掌、指侧固有动脉(PPDA)、指背动脉弓(DDAA)及甲床三级动脉弓(FN3AA)血管结构,记录血流动力学参数,并行组间比较。结果 男性组各动脉内径(D)均大于女性组(P均<0.05);BA、PPDA、DDAA及FN3AA血流阻力指数(RI)低于均女性组(P均<0.05);BA、UA、RA及PPDA每分钟血流容积(VFlow)均高于女性组(P均<0.05)。男性组BA单相波比例高于女性组(P<0.05)。2组BA、RA、PPDA动脉间单相波形占比、D、VFlow、RI、收缩期峰值流速及其加速度差异均有统计学意义(P均<0.05),随动脉由近心端(BA)延伸至远心端(PPDA),单相波形占比逐渐增高,其余上述参数逐渐减低(P均<0.05)。结论 健康成人上肢动脉血流动力学及血管结构存在性别差异。 |

Objective To investigate the hemodynamic changes of the upper limb artery from brachial artery (BA) to fingertip arterioles in healthy adults and the differences between genders. Methods A total of 205 healthy volunteers were enrolled and divided into male group (n=88) and female group (n=117). The artery structure and hemodynamic parameters of the right upper limb were obtained with conventional color Doppler ultrasound combined with angio planewave ultrasensitive imaging (Angio PLUS)and compared between groups, including BA, ulnar artery (UA), radial artery (RA), proper palmar digital artery (PPDA), dorsal digital artery arch (DDAA) and the third grade artery arch of fingernail (FN3AA). Results The inner diameter (D) of arteries in male group were all larger than that in female group (all P<0.05), while the blood flow resistance index (RI) of BA, PPDA, DDAA and FN3AA in male group were lower, and the blood flow volume (VFlow) of BA, RA, UA and PPDA were higher than those in female group (all P<0.05). The monophasic wave percentage of BA was higher in male group than tin female group (P<0.05). There were significant differences of the percentage of monophasic wave, D, VFlow, RI, peak systolic velocity and acceleration of BA, RA and PPDA between groups (all P<0.05). The proportion of monophasic waveform gradually increased with the extension of artery from the proximal (BA) to the distal (PPDA), while the other parameters gradually decreased (all P<0.05). Conclusion Gender differences of hemodynamics and vascular structures of upper limb arteries existed in healthy adults. |
