      目的 探讨Ⅰ期肺腺癌18F-FDG PET/CT表现与Ki-67表达的相关性。方法 回顾性分析56例Ⅰ期肺腺癌患者的18F-FDG PET/CT及病理学资料,将其分为纯磨玻璃结节(pGGN)组(15例)、混杂磨玻璃结节(mGGN)组(11例)及实性结节组(30例),观察其PET/CT征象与Ki-67表达之间的相关性。结果 实性结节组SUVmax明显高于mGGN组和pGGN组(P均<0.05),mGGN组SUVmax明显高于pGGN组(P<0.05);实性结节组Ki-67明显高于mGGN组和pGGN组(P均<0.05)。Ⅰ期肺腺癌Ki-67阳性表达与SUVmax呈正相关(P<0.05),与GGO百分比及毛刺征均呈负相关(P均<0.05),而与肿瘤体积、分叶征、血管集束征、胸膜凹陷征及支气管截断征均无明显相关(P均>0.05)。SUVmax与肿瘤体积呈正相关(P<0.05),与GGO百分比呈负相关(P<0.05),与毛刺征、血管集束征、胸膜凹陷征及支气管截断征均存在相关性(P均<0.05),与分叶征无明显相关(P>0.05)。结论 Ⅰ期肺腺癌Ki-67阳性表达与18F-FDG PET/CT所示SUVmax及肿瘤实性成分比例正相关;SUVmax与毛刺征、胸膜凹陷征、血管集束征及支气管截断征均存在相关性。

      Objective To investigate the correlation of 18F-FDG PET/CT manifestations and the expression of Ki-67 in stage Ⅰ lung adenocarcinoma. Methods 18F-FDG PET/CT and pathological data of 56 patients with stage Ⅰ lung adenocarcinoma were retrospectively analyzed. The nodules were divided into pure ground-glass nodules (pGGN) group (n=15), mixed ground-glass nodules (mGGN) group (n=11) and solid nodules group (n=30). The correlation of PET/CT manifestations, including maximum standardized uptake value (SUVmax), nodule volume, percentage of ground-glass opacity (GGO), spiculation sign, lobulation sign, pleural indentation sign, vessel convergence sign and bronchial truncation sign with Ki-67 were analyzed. Results SUVmax was significantly higher in solid nodules group than that in mGGN group and pGGN group (both P<0.05), and in mGGN group was higher than in pGGN group (P<0.05). The expression of Ki-67 was significantly higher in solid nodules group than tin mGGN group and pGGN group (both P<0.05). Positive expression of Ki-67 positively correlated with SUVmax and negatively correlated with GGO percentage (both P<0.05), but not correlated with nodule volume, lobulation sign, vessel convergence sign, pleural indentation sign nor bronchial truncation sign (all P>0.05). SUVmax positively correlated with nodule volume and negatively correlated with GGO percentage (all P<0.05), while not correlated with spiculation sign, vessel convergence sign, pleural indentation sign nor bronchial truncation sign (all P<0.05). Conclusion Ki-67 positive expression of stage Ⅰ lung adenocarcinoma were positively correlated with 18F-FDG PET/CT SUVmax and the ratio of solid composition within the tumor. SUVmax of stageⅠlung adenocarcinoma correlated with spiculation sign, pleural indentation sign, vessel convergence sign and bronchial truncation sign.
