      目的 探讨超声诊断和鉴别诊断女性乳头疾病的价值。方法 回顾性分析25例超声首诊乳头异常患者,其中16例经病理证实,4例经随访证实,5例为临床诊断,分析不同乳头疾病的超声特征。结果 16例中,乳头佩吉特病9例,均经病理证实,包括混合型6例、单纯型3例,超声诊断准确率88.89%(8/9);乳头炎症6例,2例经手术活检证实、4例经随访证实,超声诊断准确率66.67%(4/6);导管内乳头状瘤4例,超声诊断准确率100%(4/4);乳头内导管原位癌1例,超声缺乏典型特征;另发现乳头内钙化灶3例,乳头内囊性结节2例。结论 乳头疾病多与导管有关。超声结合临床可为诊断和鉴别诊断乳头疾病提供重要信息。
英文摘要:
      Objective To explore the diagnostic and differential diagnostic value of ultrasound of female nipple diseases. Methods Totally 25 abnormal nipples patients detected with the first time ultrasound were retrospectively analyzed. Among them, 16 cases were confirmed by pathology, 4 were confirmed by following-up, 5 cases were clinically diagnosed. The ultrasonic characteristics of different nipple diseases were observed. Results Paget disease was confirmed with pathology in 9 cases, including 6 cases of mixed type and 3 cases of simple type, and the diagnostic accuracy of ultrasound was 88.89% (8/9). Inflammation of the nipples were noticed in 6 cases, 2 were confirmed with surgical biopsy and 4 by following-up, the diagnostic accuracy of ultrasound was 66.67% (4/6). There were 4 cases of intraductal papilloma, and the diagnostic accuracy of ultrasound was 100% (4/4). There was 1 case of ductal carcinoma in situ without typical ultrasonic features. Calcifications were detected in 3 cases, 2 were found with cystic nodules. Conclusion Most nipple diseases related with mammary ducts. Combining with clinical information,ultrasound could provide important information for diagnosis and differential diagnosis for nipple diseases.
