目的 观察海马体和皮层基底神经节回路之间的解剖与功能连接(FC),探讨肝豆状核变性(WD)神经心理学症状的致病机制。方法 收集25例WD患者(WD组),以26名年龄、性别相匹配的健康志愿者为对照组,采集功能MRI和弥散张量成像;利用功能神经影像分析(AFNI)和脑功能成像软件包(FSL),以双侧海马为ROI,计算全脑FC并分析组间差异;以差异有统计学意义的脑区作为ROI,提取相应部位的各向异性分数(FA),将其与简易精神状态检查(MMSE)、数字广度测验(DST)及词语流畅性测验(VFT)进行协变量相关分析。采用t检验分析,并进行FDR校正。结果 WD组左侧豆状核、尾状核头和双侧丘脑FC较对照组减弱,右侧颞叶、左侧额叶FC增强;WD组丘脑(THA)和左侧海马(LM) FA低于对照组,左侧基底核区(LB) FA高于对照组。MMSE评分与右侧额中回、颞横回FC强度呈正相关;VFT评分与左侧豆状核、右侧额中回及双侧颞横回FC强度呈正相关,VFT评分与双侧丘脑和双侧豆状核FA呈负相关。结论 WD患者左侧海马和双侧丘脑、左侧豆状核及尾状核头之间功能和解剖学连接性发生变化,使通路连接受阻,最终导致认知功能障碍。 |

Objective To elucidate anatomical and functional connectivity (FC) among hippocampus and the cortical-basal ganglia circuits, in order to explore the pathogenic mechanisms for neuropsychological performances of Wilson disease (WD). Methods A total of 25 WD patients (WD group) and 26 age- and sex-matched healthy volunteers (control group) were enrolled. Functional MRI and diffusion tensor imaging sequences were acquired. Analysis of functional neuroimage (AFNI) and functional MRI of the brain software library (FSL) were used to calculate the whole brain FC taken bilateral hippocampus as ROIs and analyze the differences between groups. The brain regions with differences between groups were used as ROIs to extract fractional anisotropy (FA) of corresponding parts. Then covariate correlation analysis was performed between FA of each part and mini-mental state examination (MMSE), digital span test (DST) and verbal fluency test (VFT), respectively. Then t test analysis and false discovery rate (FDR) correction were performed. Results FC of left lenticular nucleus, head of caudate nucleus and the bilateral thalamus in WD group were significantly weaker, while of right temporal lobe and left frontal lobe were stronger than those in control group. FA of thalamus (THA) and left hippocampus (LM) in WD group were lower, of left basal ganglia (LB) was higher than that in control group. MMSE score was positively correlated with FC of right middle frontal gyrus and transverse temporal gyrus. VFT score was positively correlated with FC of left lenticular nucleus, right middle frontal gyrus and bilateral transverse temporal gyrus, while VFT score was negatively correlated with FA of bilateral thalamus and bilateral lenticular nucleus. Conclusion The functional and anatomical connectivity changes of the hippocampus and bilateral thalamus, left lenticular nucleus, head of caudate nucleus in WD patients caused blocked pathway connections and eventually cognitive dysfunction. |
