目的 比较高和超高b值弥散加权成像(DWI)图像质量和显示直肠癌效果,优化选择b值。方法 对58例直肠腺癌患者行多个b值(800、1 000、1 500、2 000 s/mm2)DWI扫描(DWI800、DWI1 000、DWI1 500、DWI2 000),对比图像质量和病变信号强度分级,测量并比较不同b值下肿瘤信噪比(SNR)、对比度噪声比(CNR)和信号强度比(SIR)。结果 DWI800、DWI1 000、DWI1 500及DWI2 000图像质量评分差异均有统计学意义(P<0.001),且DWI800、DWI1 000、DWI1 500图像质量评分呈升高趋势,DWI1 000与DWI1 500间差异有统计学意义(P<0.001)。DWI800、DWI1 000、DWI1 500图像信号强度为Ⅰ型者逐渐增多、Ⅱ型逐渐减少(P均<0.001),DWI1 500图像中无Ⅲ型信号;DWI2 000图像中Ⅰ型信号略少于DWI1 500,差异无统计学意义(P=0.096)。随b值升高,SNR、CNR和SIR均逐渐降低(P均<0.001)。结论 b值为800、1 000、1 500、2 000 s/mm2的DWI图像中,b=1 500 s/mm2时图像质量最优,显示直肠癌病灶最佳。 |

Objective To compare image quality and display effect of rectal cancer with diffusion weighted imaging (DWI) using high and ultrahigh b value, so as to optimize b value selection. Methods Multiple b values (800, 1 000, 1 500 and 2 000 s/mm2) DWI scanning (DWI800,DWI1 000,DWI1 500 and DWI2 000) were performed on 58 patients with rectal adenocarcinoma. The imaging quality, grading signal intensity, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and signal intensity ratio (SIR) were measured and compared among DWI images obtained using different b values. Results The image quality scores of DWI800, DWI1 000, DWI1 500 and DWI2 000 were statistically different (P<0.001). The image quality scores of DWI800, DWI1 000 and DWI1 500 showed an increasing trend, and the difference between DWI1 000 and DWI1 500 was statistically significant (P<0.001). On DWI800, DWI1 000, and DWI1 500 images, type Ⅰ signal lesions increased while type Ⅱdecreased gradually (all P<0.001), and there was no type Ⅲ signal on DWI1 500. The number of type Ⅰ signal lesions on DWI2 000 was slightly fewer than that of DWI1 500, but there was not statistical difference (P=0.096). With the increase of b value, SNR, CNR and SIR all gradually decreased (all P<0.001). Conclusion Among DWI images with b value of 800, 1 000, 1 500 and 2 000 s/mm2, image quality of b=1 500 s/mm2 was the best, and displaying of rectal cancer lesions was the best. |
