目的 利用高频超声测量健康人双侧股直肌不同体位下的形态学指标,将超声参数与等速肌力测量参数对比,探讨高频超声对健康人肌力评估的可行性及应用价值。方法 纳入60名四肢肌力5级的健康志愿者,用高频超声测量双侧股直肌在放松位、紧张位、负重1 kg以及负重2 kg的4种体位下的形态学指标,包括股直肌的宽度、厚度、横截面积及羽状角。用等速肌力测试仪测量双侧股四头肌在10次最大用力伸膝动作过程中得出的峰力矩(PT)、峰力矩体质量比(PT/BW)以及总功(TW)。结果 4种体位下,股直肌的宽度、厚度、横截面积及羽状角的差异均有统计学意义(P均<0.001)。放松位下,男性与女性股直肌的厚度、横截面积及羽状角均有统计学意义(P均<0.05),用力后男性与女性上述指标放松位与紧张位的差值,股直肌的宽度、厚度及横截面积的差异有统计学意义(P均<0.001)。紧张位下,股直肌厚度与PT、PT/BW及TW呈正相关(r=0.495、0.391、0.508,P均<0.05);横截面积与PT、TW呈正相关(r=0.221、0.315,P均<0.05);宽度及羽状角与PT、PT/BW及TW无明显相关性(P均>0.05)。结论 高频超声能够发现不同状态下股直肌形态学参数的差异,并受性别影响,与等速肌力测试参数有一定相关性,厚度及横截面积可能是评估肌力的较好参数。 |

Objective To measure morphological indexes of healthy bilateral rectus femoris under different positions with means of high-frequency ultrasound, and to compare the morphological indexes of ultrasound and the parameters of isokinetic muscle test, to discuss the feasibility and application value of high-frequency ultrasound on the strength assessment of healthy people. Methods Sixty healthy volunteers with grade 5 limbs myodynamia underwent high-frequency ultrasound, and morphological indexes of bilateral rectus femoris under four kinds of postures of relaxed, tension, weight 1 kg and weight 2 kg were measured. The morphological indexes included width, thickness, cross-sectional area and the pinnate angle. Bilateral quadriceps in 10 times maximum effort knee extensionwas measured with isokinetic muscle test and the peak torque (PT), peak torque weight ratio (PT/BW), and total work (TW) were recorded. Results The width, thickness, cross-sectional area and the pinnate angle of the rectus femoris were statistically significant among four different positions (all P<0.001). The thickness, cross-sectional area and pinnate angle of the rectus femoris under relaxed position were statistically significant between men and women (all P<0.05). The difference value of relaxed and tension of above indicators, and the width, thickness, cross-sectional area were statistically significant between men and women under the tension position (all P<0.001). Under the tension position, the thickness of the femoral muscle was positively correlated with PT, PT/BW and TW (r=0.495, 0.391, 0.508, all P<0.05). The cross-sectional area was positively correlated with PT and TW (r=0.221, 0.315, all P<0.05). There was no significant correlation between the width and the pinnate angle with PT, PT/BW and TW (both P>0.05). Conclusion High-frequency ultrasound can find the differences of rectus muscle morphological parameters under different state, which is influenced by gender. The morphological parameters have a certain correlation with the isokinetic muscle testing parameters. Thickness and cross-sectional area are good parameters for evaluating muscle strength. |
