| 目的 分析肝脏血管平滑肌脂肪瘤的CT及MRI征象特点。方法 回顾性分析经手术病理证实的7例肝脏血管平滑肌脂肪瘤患者的CT和/或MRI表现，并与病理结果进行对照。结果 CT及MRI平扫共8个病灶，CT显示6个病灶中，5个呈稍低密度，1个病灶大部分呈低密度，其内可见小片状等密度。MR显示的4个病灶中，2个脂肪抑制T2WI呈低信号，T1WI呈较均匀低信号，1病灶未显示脂肪信号特点，病灶内未见囊变、血肿、坏死等信号。增强扫描动脉期7个病灶表现为明显不均匀强化，1个病灶呈均匀强化；3个病灶呈"快进快出"强化改变，5个病灶呈持续强化改变；3个病灶内部见粗大畸形血管，5个病灶可见引流静脉。结论 肝脏血管平滑肌脂肪瘤的影像学表现有一定特征性，影像学检查有助于诊断和鉴别诊断。 |

| Objective To analyze the CT and MRI features of hepatic angiomyolipoma. Methods CT and/or MRI findings of 7 patients with hepatic angiomyolipoma confirmed by operation and pathology were retrospectively analyzed and compared with pathological findings. Results There were 8 lesions on plain CT and MRI scans. Of the 6 lesions in CT, 5 showed slightly low density, and the 1 lesion showed most of low density, with small patches of isodensity. Among the 4 lesions in MR, 2 lesions showed low signal on fat suppression T2WI and homogeneous low signal on T1WI, 1 lesions did not show fat signal characteristics, and no signal on cystic degeneration, hematoma and necrosis was found. On contrast-enhanced scan, 7 lesions showed marked heterogeneous enhancement, 1 lesion showed uniform enhancement, 3 lesions showed "fast-in and fast-out" enhancement, 5 lesions showed continuous enhancement, 3 lesions showed large malformed vessels and 5 lesions showed drainage veins. Conclusion The imaging manifestations of hepatic angiomyolipoma have certain characteristics. Imaging examination is helpful for the diagnosis and differential diagnosis of hepatic angiomyolipoma. |
