      目的 探讨新生儿体质量对初产妇耻骨直肠肌(PR)弹性和形态的影响。方法 收集产后6~8周的65名足月单胎且经阴道自然分娩的初产妇,根据新生儿出生体质量将其分为新生儿体质量正常组(A组,新生儿体质量为2.5~<4.0 kg,n=50)和巨大儿组(B组,新生儿体质量≥ 4.0 kg,n=15)。收集同期健康未育女性60名为对照组。分别于静息及最大收缩状态下以腔内超声测量PR前部的厚度,以剪切波弹性成像技术(SWE)测量PR前部的杨氏模量,并分别计算差值。比较3组间PR厚度和杨氏模量的差异。结果 A组、B与对照组间静息和最大收缩状态下双侧PR厚度及收缩前后差值、杨氏模量及收缩前后差值总体差异均有统计学意义(P均<0.05),且对照组、A组、B组依次减低,两两比较差异均有统计学意义(P均<0.05)。结论 初产妇产后PR的弹性和形态同步改变;妊娠和分娩巨大儿更易造成初产妇PR弹性降低,厚度变薄,收缩功能减低。

      Objective To explore the impact of newborn weight on the elasticity and morphology of puborectalis (PR) in primiparas. Methods Totally 65 primiparas 6-8 weeks postpartum who underwent full-term single fetus birth with spontaneous vaginal delivery were enrolled. The primiparas were divided into the normal newborn weight group (group A, newborn weight 2.5-<4.0 kg, n=50) and the macrosomia group(group B, newborn weight ≥ 4.0 kg, n=15)according to the newborn birth weight. At the same time, 60 healthy nulliparous women were selected as the controls (control group). The thickness of the anterior region of PR was measured with transvaginal ultrasonography, the Young's modulus of the anterior region of PR was measured with shear wave elastography (SWE) at rest and maximal contraction state respectively, and the difference was calculated separately. The differences of PR thickness and Young's modulus were compared among the three groups. Results At rest and maximal contraction state, bilateral PR thickness, Young's modulus and the differences between contraction and rest among the three groups were statistically significant (all P<0.05). PR thickness and Young's modulus in the control group, group A and B reduced in turn, and the differences were statistically significant between each two groups (all P<0.05). Conclusion The elasticity and morphology of PR changed synchronously in primiparas after birth. Pregnancy and delivery macrosomia are more likely to cause decreased elasticity, thickness and contract function of PR in the primiparas.
