      目的 探讨肝细胞癌(HCC)钆贝葡胺(Gd-BOPTA)增强MRI肝胆期强化与病理学分级及免疫组化指标间的关系。方法 回顾性分析接受Gd-BOPTA增强MR扫描的53例HCC患者的MRI及病理学资料,测量HCC肝胆期强化率(CER)、SNR和CNR。依据术后病理分级及免疫组织化学标志物CK7、CK19、CD34、Ki-67的表达进行分组,比较组间差异。结果 病理分级Ⅰ级HCC患者11例、Ⅱ级27例、Ⅲ级15例,无Ⅳ级患者;不同病理分级HCC之间Gd-BOPTA增强MRI肝胆期CER、SNR和CNR差异均无统计学意义(P均>0.05)。CK19阳性组与阴性组、CD34阳性组与阴性组HCC之间肝胆期CER差异有统计学意义(P均<0.05),SNR和CNR差异无统计学意义(P均>0.05);CK7和Ki-67阳性组与阴性组间肝胆期CER、SNR和CNR差异均无统计学意义(P均>0.05)。结论 HCC的Gd-BOPTA增强MRI肝胆期CER与CK19或CD34阳性表达有一定关联,肝胆期CER越低,越倾向于预后不良。

      Objective To investigate the correlation of hepatobiliary phase enhancement of Gd-BOPTA-enhanced MRI with pathological grades and immunohistochemical indexes of hepatocellular carcinoma (HCC). Methods Imaging and pathological data of 53 HCC patients underwent Gd-BOPTA-enhanced MR were retrospectively analyzed. The contrast enhancement ratio (CER), SNR and CNR in hepatobiliary phase were measured. The patients were grouped according to the pathological grades and the expression of immunohistochemical markers of CK7, CK19, CD34 and Ki-67, and the differences were compared among these groups. Results There were 11 patients of grade Ⅰ, 27 of grade Ⅱ, 15 of grade Ⅲ and none of grade Ⅳ, and no significant difference of CER, SNR nor CNR in hepatobiliary phase of Gd-BOPTA-enhanced MRI was detected among these pathological grades (all P>0.05). Between the positive group and negative group of CK19 and CD34, there were significant differences in CER in hepatobiliary phase (both P<0.05), while no significant difference of SNR nor CNR was found (all P>0.05). Between the positive and negative groups of CK7 and Ki-67, there was no significant difference of CER, SNR nor CNR in hepatobiliary phase (all P>0.05). Conclusion CER of HCC in hepatobiliary phase of Gd-BOPTA-enhanced MRI is correlated with the expression of CK19 or CD34 to some extent. The lower the CER in hepatobiliary phase, the more prone to poor prognosis.
