      目的 采用Meta分析评价18F-FDG PET/CT评价胃肠间质瘤(GIST)恶变潜能的价值。方法 检索PubMed、EMBASE、Cochrane图书馆、中国知网和中国生物医学数据库中有关18F-FDG PET/CT评价GIST恶变的中英文文献,检索时间截止2017年12月。由2名研究者进行数据提取和质量评价后,采用Meta-Disc软件进行统计分析。结果 共纳入6项研究、157例患者。Meta分析结果显示,合并敏感度、合并特异度、合并阳性似然比、合并阴性似然比及合并诊断比值比分别为[95%CI(0.76,0.92)]、0.79[95%CI(0.68,0.87)]、3.35[95%CI(1.81,6.20)]、0.22[95%CI(0.13,0.37)],汇总ROC(SROC)曲线下面积为0.9057(P<0.05)。Deek's漏斗图提示存在发表偏倚的可能性较小。亚组分析和Meta回归分析结果显示诊断试验的准确率基本不受设备型号和SUVmax临界值的影响。结论 18F-FDG PET-CT评价胃肠间质瘤恶变潜能具有较高诊断效能。

      Objective To explore the value of 18F-FDG PET/CT in evaluation on the malignant potential of gastrointestinal stromal tumors (GIST) with Meta-analysis. Methods The database of PubMed, EMBASE, Cochrane Library, CNKI and CBM were researched from establishment to December 2017, for the diagnostic studies about assessing the malignant potential of GIST using 18F-FDG PET/CT. The data were extracted, and the quality of included studies was assessed by 2 reviewers, and then were analyzed with Meta-Disc software. Results Totally 6 studies involving 157 patients were included. Meta-analysis showed that the pooled sensitivity, pooled specificity, pooled positive likelihood ratio, pooled negative likelihood ratio and diagnostic odds ratio was 0.85(95%CI[0.76, 0.92]), 0.79 (95%CI[0.68, 0.87]), 3.35 (95%CI[1.81, 6.20], 0.22 (95%CI[0.13, 0.37]) and 18.28 (95%CI[6.31, 52.95]), respectively, and the AUC of summary ROC (SROC) was 0.905 7 (P<0.05). Deek's funnel plot showed that the possibility of publication of bias was small. The subgroup analysis and Meta-regression results indicated that the equipment model and the threshold of SUVmax did not obviously influence the accuracy of 18F-FDG PET/CT. Conclusion There was high diagnostic efficacy for 18F-FDG PET/CT in assessing the malignant potential of GIST.
