目的 评估在疑似泌尿系统结石患者CT扫描中沿z轴过度扫描现象发生的频率及程度,探讨导致该现象的主要的因素及相应解决方案。方法 回顾性分析天津市3所三级甲等医院共172例疑诊泌尿系统结石患者的CT扫描资料,测量实际扫描距离(L0)、有效扫描距离(L1),计算额外扫描距离(Lex)、额外辐射剂量占实际辐射剂量的百分比(EDex%)和过度扫描发生率,分析比较不同医疗机构和患者类别(急诊、门诊及住院患者)间的差异。结果 本组172例患者中,169例存在过度扫描,发生率为98.26%(169/172)。不同患者类别间Lex差异有统计学意义(P=0.002);3所医疗机构间Lex和EDex%差异均有统计学意义(P均<0.001)。结论 对疑似泌尿系统结石患者行CT扫描时,沿z轴过度扫描现象十分普遍;通过训练影像技师,可减轻或避免这种现象。 |

Objective To evaluate the frequency and degree of over scanning along z-axis in CT scanning of patients with suspected urolithiasis, and to explore the main factors that lead to this phenomenon and the corresponding solutions. Methods CT scanning data of 172 patients with suspected urolithiasis from 3 third-level grade-A hospitals of Tianjin were retrospectively analyzed. The actual scanning distance (L0) and the effective scanning distance (L1) were obtained. The extra scanning distance (Lex), the percentage of the extra radiation dose to the actual radiation dose (EDex%) and the incidence of over scanning were calculated. The differences among different medical institutions and patient settings (emergency, outpatient and inpatient) were analyzed. Results In all 172 patients, 169 patients were over scanned, and the incidence was 98.26% (169/172). Statistically significant differences of Lex were found among different patient settings (P=0.002). The differences of Lex and EDex% among 3 medical institutions were statistically significant (both P<0.001). Conclusion Over scanning along z-axis in CT scanning of patients with suspected urolithiasis is a common phenomenon, which can be reduced or avoided by training of the imaging technicians. |
