      目的 探讨采用宽体探测器CT进行心脑血管CTA"一站式"扫描的价值。方法 回顾性分析73例接受心脑血管CTA扫描患者的图像,根据扫描方法分为A组和B组。A组(n=32)注射1次对比剂行冠状动脉CTA扫描后立即行头颈部CTA扫描;B组(n=41)注射2次对比剂分别行冠状动脉CTA和头颈部CTA扫描。对2组患者的图像质量进行主观评分和客观评价,比较其图像质量、辐射剂量和对比剂用量。结果 A组和B组CTA图像质量均可满足诊断要求,且冠状动脉CTA和头颈部CTA图像质量主观评分差异均无统计学意义(P均>0.05)。A组与B组冠状动脉和头颈部CTA中,右冠状动脉、左前降支、回旋支和颈总动脉、颈内动脉、大脑中动脉CT值差异均无统计学意义(P均> 0.05),冠状动脉CTA的噪声和CNR、头颈部CTA的噪声差异有统计学意义(P均<0.05)。A组和B组对比剂用量分别为(51.00±6.69)ml和(105.41±14.56)ml,差异有统计学意义(P < 0.001)。2组CT容积剂量指数和有效剂量差异均无统计学意义(P均>0.05)。结论 宽体探测器CT应用于心脑血管CTA"一站式"扫描时,图像质量可满足诊断要求,且能明显降低对比剂用量。

      Objective To explore the value of "one-stop" coronary CT angiography (CCTA) and head and neck CTA using wide detector CT. Methods CTA images of 73 patients who underwent CCTA and head and neck CTA were retrospectively analyzed. The patients were divided into group A and group B according to the CTA examination methods. CCTA was performed, and immediately followed by head and neck CTA with one injection of contrast agent in group A (n=32), while CCTA and head and neck CTA were performed separately with twice injection of contrast agent in group B (n=41). Subjective evaluation and objective evaluation were performed on the image quality of the two groups. Then image quality, radiation dose and contrast agent dose were compared between the two groups. Results The image quality all met the diagnostic requirements in the two groups, and there was no difference of subjective image quality between the two groups (all P>0.05). There was no statistical difference in CT value of the right coronary artery, left anterior descending, left circumflex artery, common carotid artery, internal carotid artery nor middle cerebral artery between the two groups (all P>0.05). There was significantly different of standard deviation and CNR in CCTA and standard deviation in head and neck CTA between the two groups (all P<0.05). The average contrast amount in group A was (51.00±6.69)ml, significantly less than that in group B ([105.41±14.56]ml, P<0.001). There was no significant difference of radiation dose between the two groups (P>0.05). Conclusion The "one-stop" CCTA and head and neck CTA in one contrast injection using wide detector CT provides diagnostically acceptable images while significantly reducing contrast agent dose.
