      目的 探讨第一跖骨头下腓侧籽骨与外翻的相关性。方法 对2 000例正常足(右侧足入组,对照组)及60例外翻患者(双侧外翻者取严重侧入组,外翻组)拍摄足正、斜位片,测量以下参数:外翻角(HVA),第一、二跖骨间角(IMA),第二跖骨长度(CD),腓侧籽骨位置(FSP),腓侧籽骨至第一跖骨中线长轴距离(EF)和胫侧籽骨与腓侧籽骨距离(GH),并计算EF与CD的比值(K1)和GH与CD的比值(K2)。采用多重线性回归分析FSP、K1、K2与HVA的线性相关关系,并获得线性回归方程。结果 外翻组与对照组间FSP、K1和K2值差异均有统计学意义(P均<0.05)。所有受试者中,FSP、K1、K2与HVA具有相关性,其线性回归方程为HVA=63.976-10.541FSP-0.446K1-1.115K2。在HVA≥20°的受试者(n=107)中,FSP与HVA呈中度负相关(r=-0.494,P<0.05),线性回归方程为HVA=27.720-6.028FSP;外翻组中,FSP与HVA呈高度负相关(r=-0.824,P<0.05),其线性回归方程为HVA=26.842-6.011FSP。结论 第一跖骨头下腓侧籽骨脱位是导致外翻的重要病理因素之一,FSP与HVA呈负相关。

      Objective To explore the relationship between the first metatarsal head fibular sesamoid bone and hallux valgus. Methods Totally 2 000 patients of normal foot (the right side, control group) and 60 patients of hallux valgus (the severe side was enrolled if bilateral lesions, hallux valgus group) underwent anteroposterior and oblique X-ray photograph. The hallux valgus angle (HVA), intermetatarsal angle (IMA), the second metatarsal length (CD), the fibular sesamoid position (FSP), the distance of the fibular sesamoid to first metatarsal midline long axis (EF) and the distance between the tibial seed bone and fibular sesamoid bone (GH) were measured, then the ratios of EF to CD (K1) and GH to CD (K2) were calculated. Multiple linear regression was used to analyze the linear correlation of FSP, K1, K2 and HVA, and the linear regression equation was obtained. Results The differences of FSP, K1 and K2 were statistically significant between the hallux valgus group and the control group (all P<0.05). In all subjects, the FSP, K1 and K2 were correlated with HVA, and the linear regression equation was HVA=63.976-10.541FSP-0.446K1-1.115K2. In subjects with HVA ≥ 20° (n=107), FSP was moderately negatively correlated with HVA (r=-0.494, P<0.05), and the linear regression equation was HVA=27.720-6.028FSP. In hallux valgus group, FSP was highly negatively correlated with HVA (r=-0.824, P<0.05), and the linear regression equation was HVA=26.842-6.011FSP. Conclusion The dislocation of the fibular sesamoid bone on the first metatarsal head is one of the important pathological factors leading to hallux valgus, and there is negative correlation between FSP and HVA.
