目的 评价动态对比增强MRI (DCE-MRI)定量参数容积转运常数(Ktrans)、血管外细胞外容积分数(Ve)和血浆容积分数(Vp)分级诊断脑胶质瘤的价值。方法 于PubMed、Embase、CNKI、万方、维普数据库检索截止至2017年10月发表的有关DCE-MRI对胶质瘤分级诊断的中英文文献,以Meta分析方法评价DCE-MRI对胶质瘤的分级诊断效能。结果 共纳入27篇文献(中文13篇,英文14篇)、1 079例患者。DCE-MRI定量参数Ktrans、Ve、Vp对胶质瘤分级诊断的汇总敏感度为0.86、0.85、0.81,汇总特异度为0.83、0.85、0.79,对应汇总ROC曲线的AUC为0.91、0.92、0.86。结论 Ktrans、Ve、Vp对胶质瘤术前分级诊断具有较高诊断效能,其中Ktrans、Ve可信度较高。 |

Objective To explore the value of quantitative parameters Ktrans, Ve and Vp derived from dynamic contrast-enhanced MRI (DCE-MRI) in grading of brain gliomas. Methods A comprehensive searching of PubMed, Embase, CNKI, Wanfang and Vip databases was performed up to October, 2017. All English and Chinese literatures on the value of DCE-MRI in grading diagnosis of gliomas were included. Meta-analysis was used to evaluate the diagnostic efficacy of DCE-MRI parameters for glioma grading. Results Totally 27 studies (14 English, 13 Chinese) including 1 079 patients were enrolled. The summary sensitivity of quantitative parameters Ktrans, Ve, Vp derived from DCE-MRI was 0.86 (95%CI), 0.85 (95%CI) and 0.81 (95%CI), the summary specificity was 0.83 (95%CI), 0.85 (95%CI) and 0.79 (95%CI), respectively. AUC of summary ROC curve was 0.91, 0.92 and 0.86, respectively. Conclusion Ktrans, Ve and Vp have high efficacy in grading diagnosis of glioma, which is helpful to the preoperative diagnosis, and the reliability of Ktrans and Ve are good. |
