| 目的 观察起源于肾周脂肪组织的原发性腹膜后脂肪肉瘤（PRPL）的CT征象。方法 回顾性分析经手术病理证实的8例PRPL术前MDCT表现。结果 8例患侧肾脏被包绕及肾前间隙受侵，1例右肾前间隙受累，2例双侧肾前间隙均受累，5例左肾前间隙受累；3例患侧肾受压推移发生轴位旋转。MDCT表现：3例肿块内可见粗颗粒状钙化，8例均可见脂肪成分，多期增强扫描钙化及脂肪成分均无强化；6例肿块内脂肪成分中可见分界清楚、大小不等的结节状软组织成分，多期增强扫描呈轻-中度持续性强化，病理为去分化成分或黏液成分；1例由同侧肾上腺动脉供血，3例由肠系膜上动脉供血，4例由肠系膜下动脉供血。8例均呈塑形生长，可见完整或部分完整的包膜，多期增强扫描包膜呈轻-中度持续性强化，病理为纤维结缔组织成分；2例延伸至整个盆腔。结论 病灶内脂肪密度区、强化的实性结节及患侧肾被包绕及肾前间隙受侵高度提示PRPL。 |

| Objective To observe manifestations of primary retroperitoneal liposarcoma(PRPL)originated from perirenal fatty tissue in CT. Methods Imaging features of histopathologically proved PRPL in 8 patients were retrospectively analyzed. Results Ipsilateral kidney were surrounded with anterior pararenal space invaded in 8 cases, right anterior pararenal space invaded in 1 case, both anterior pararenal space invaded in 2 cases and left anterior pararenal space invaded in 5 cases. Ipsilateral kidney were compressed and axial rotated in 3 cases. Coarse calcifications were found in 3 cases, which were showed no enhacement. Fatty components with distinct demarcation and different sizes were found in 8 cases, which were showed slight-moderate post-contrast enhancement in multi-phase enhanced scan, and dedifferentiation or mucus were confirmed by histopathology. Feeding arteries originated from adrenal artery branches in 1 case, superior mesenteric artery branches in 3 cases and inferior mesenteric artery branches in 4 cases. Shaped growth and intact or incomplete capsule with slight-moderate post-contrast enhancement were found in 8 cases, which were confirmed fibrous connective tissue by histopathology. Extended to the pelvic cavity in 2 cases. Conclusion Fat density, non-fat-containing nodules with enhancement, capsule with the ipsilateral kidney surrounded and anterior pararenal space invaded are highly prompt of PRPL. |
