| 目的 探讨高分辨MRI （HR-MRI）对直肠癌术前TN分期及手术方式选择的价值。方法 对104例直肠癌患者行直肠高分辨MRI检查，进行T分期和N分期，拟定手术方式，并与病理和最终手术方式对照。结果 104例直肠癌患者中，97例手术方式符合最终手术方案，术式符合率为93.27%（97/104），58例中高位直肠癌拟定术式符合率为98.28%（57/58），46例低位直肠癌拟定术式符合率为86.96%（40/46），拟定术式与实际术式的差异无统计学意义（P > 0.05）。直肠癌MRI表现T分期符合率93.27%（97/104），N分期符合率67.31%（70/104）。结论 直肠高分辨MRI在直肠癌T分期及拟定手术方式准确率高，对术前手术方案制定有重要参考价值。 |

| Objective To explore the value of high resolution MRI (HR-MRI) in TN staging and surgical selecting of rectal cancer. Methods Totally 104 patients with rectal cancer were performed HR-MRI. According to HR-MRI, the pre-operative T, N staging and surgical plan were performed, then compared with final operation and pathology. Results Among 104 patients with rectal cancer, the surgical plan of 96 cases matched final operation with accuracy rate 93.27% (97/104). For 58 cases of medial-high rectal cancer, the accuracy rate was 98.28% (57/58). For low rectal cancer, the accuracy rate was 86.96% (40/46). There was no statistical difference between HR-MRI based surgical plan and final operation (P>0.05). The T staging accuracy rate was 93.27% (97/104), N staging accuracy rate was 67.31% (70/104), respectively. Conclusion Rectal HR-MRI has a high accuracy rate for T stage classification and pre-operative operation planning, which has important value for surgical procedure. |
